甲强龙辅助治疗肺炎支原体感染致过敏性紫癜患儿的临床效果观察 被引量:3

Clinical Effect of Methylprednisolone on Children with Henoch Schonlein Purpura Caused by Mycoplasma Pneumoniae Infection

摘要 目的 探讨甲强龙辅助治疗肺炎支原体感染致过敏性紫癜患儿的临床疗效。方法 选取广东省清远市佛冈县人民医院2017年10月-2019年10月收治的80例肺炎支原体感染致过敏性紫癜患儿作为研究对象,按照随机数表法将患儿分为两组,每组40例。2组患儿均行心肌酶、淀粉酶、胸片,心电图等检查,并给予两组患儿止咳,退热对症等常规治疗并消除各种致病因素。对照组行常规对症治疗给予大环内脂类抗药物阿奇霉素,观察组在对照组基础上辅以甲强龙治疗,比较两组临床疗效、临床指标(皮疹消退时间、呼吸道症状消失时间、住院时间)及血清指标的水平。结果 治疗后,观察组总有效率95.00%(38/40)高于对照组80.00%(32/40),差异具有统计学意义(P<0.05);观察组皮疹消退时间、呼吸道症状缓解时间及住院时长均短于对照组,差异具有统计学意义(P<0.05);治疗后,两组CD;(CD3T细胞)、CD;(CD4T细胞)均有所上升,且观察组高于对照组,两组患儿CD;(CD8T细胞)均降低,且观察组低于对照组,差异具有统计学意义(P<0.01)。结论 肺炎支原体感染所致过敏性紫癜患儿采用甲强龙辅助治疗效果显著,皮疹消退较快、住院时间短,可控制免疫反应增强免疫功能,值得在临床推广应用。 Objective To investigate the clinical efficacy of methylprednisolone in the treatment of children with Henoch Schonlein purpura caused by Mycoplasma pneumoniae infection.Methods 80 children with Henoch Schonlein purpura caused by Mycoplasma pneumoniae infection in our hospital from October 2017 to October 2019 were selected as the research object.The children were divided into two groups according to the random number table method,with 40 cases in each group.The children in both groups were examined by myocardial enzyme,amylase,chest film and ECG,and given routine treatment such as cough relieving,antipyretic and symptomatic treatment to eliminate various pathogenic factors.The control group was treated with routine symptomatic treatment and azithromycin,an anti lipid drug in the macrocycle,and the observation group was treated with methylprednisolone on the basis of the control group.The clinical efficacy,clinical indexes (rash regression time,disappearance time of respiratory symptoms,hospital stay) and the level of serum indexes were compared between the two groups.Results After treatment,the total effective rate in the observation group was 95.00% (38/40) higher than 80.00% (32/40) in the control group (P<0.05);The skin rash subsided time,respiratory symptom remission time and hospital stay in the observation group were shorter than those in the control group (P<0.05);After treatment,CD;(CD3t cells) and CD;(CD4T cells) in the two groups increased,and the observation group was higher than that in the control group.CD;(CD8T cells) in the two groups decreased,and the observation group was lower than that in the control group,the difference was statistically significant (P<0.01).Conclusion Methylprednisolone is effective in the adjuvant treatment of children with Henoch Schonlein purpura caused by Mycoplasma pneumoniae infection.The rash subsides quickly and the hospital stay is short.It can control the immune response and enhance the immune function.It is worthy of clinical application.
