锥形束CT图像引导用于胸部肿瘤放射治疗临床疗效观察 被引量:1

摘要 目的 分析在胸部肿瘤放射治疗中应用锥形束CT图像引导的临床价值。方法 抽选于2019年1月-2020年6月德阳市人民医院共收治的胸部肿瘤放射治疗患者70例,随机均分为两组。对照组(n=35)接受电子射野影像仪图像引导,研究组(n=35)接受锥形束CT图像引导。评价及对比两组的头脚摆位偏差、左右摆位偏差、前后摆位偏差、引导放疗时间、并发症发生情况、治疗效果。结果 研究组的头脚摆位偏差、左右摆位偏差均优于对照组(P<0.05),两组的前后摆位偏差比较无明显差异性(P>0.05);研究组的摆位时间、治疗时间均高于对照组(P<0.05);两组的图像获取时间比较无明显差异性(P>0.05);研究组的放射性肺炎率低于对照组(P<0.05)。治疗效果总有效率方面,研究组高于对照组(P<0.05)。结论 与电子射野影像仪图像引导放疗技术相比,在胸部肿瘤放射治疗中应用锥形束CT图像引导,未明显延长放射治疗总时间,但可有效避免出现严重摆位偏差,从而进一步提升放疗准确性,且可降低放射性肺炎风险,是一种安全有效的引导选择。 Objective To analyze clinical value of cone beam CT image guidance in thoracic tumor radiotherapy.Methods The paper chose 70 patients with thoracic tumor treated by radiotherapy in our hospital from January 2019 to June 2020,and divided them into two groups randomly.Control group (n=35) was treated with image guidance of electronic field imager,and study group (n=35) with image guidance of cone beam CT.Head and foot positioning deviation,left and right positioning deviation,front and rear positioning deviation,guided radiotherapy time,complications and treatment effect was evaluated and compared between two groups.Results Head foot position deviation and left-right position deviation in study group was better than control group (P<0.05).There was no significant difference in front and rear positioning deviation between two groups (P>0.05);positioning time and treatment time of study group was higher than control group (P<0.05);there was no significant difference in image acquisition time between two groups (P>0.05);radiation pneumonia rate in study group was lower than control group (P<0.05).Total effective rate of treatment effect in study group was higher than control group (P<0.05).Conclusion Compared with image-guided radiotherapy technology of electronic radiation field imager,cone beam CT image guidance in thoracic tumor radiotherapy does not prolong total time of radiotherapy significantly,it can avoid serious positioning deviation effectively,improve accuracy of radiotherapy further and reduce radiation pneumonia risks,which is a safe and effective guidance choice.
