COPD伴重症Ⅱ型呼吸衰竭老龄患者给予NIPPV并纳美芬的临床研究

Clinical Study of NIPPV Combined with Nalmefene for Elderly Patients With COPD and Severe Type Ⅱ Respiratory Failure

目的 研究COPD伴重症Ⅱ型呼吸衰竭老龄患者给予无创正压通气(NIPPV)并纳美芬的临床效果。方法 纳入研究对象选取2018年1月-2019年12月吉林中西医结合医院收治的104例患者,随机分为对照组和观察组。对照组在治疗时采取常规方法和NIPPV;观察组在治疗时也采取常规方法和NIPPV,同时还给予纳美芬。治疗前和治疗7d后对肺功能指标、血气分析指标给予对比分析。结果 治疗前两组各项肺功能指标、血气分析指标的组间对比统计结果均(P>0.05)。治疗前后两组各项肺功能指标、血气分析指标的组内对比统计结果均(P<0.05)。治疗后两组各项肺功能指标、血气分析指标的组间对比统计结果均(P<0.05)。结论 COPD伴重症Ⅱ型呼吸衰竭老龄患者给予NIPPV并纳美芬治疗在改善呼吸功能及缺氧领域作用明显。

Objective To study clinical effect of noninvasive positive pressure ventilation (NIPPV) combined with nalmefene for elderly patients with COPD and severe type Ⅱ respiratory failure.Methods The paper chose 104 patients treated in our hospital from January 2018 to December 2019,and divided them into control group and observation group randomly.Control group was treated with routine methods and NIPPV,and observation group also with routine methods,NIPPV,nalmefene during treatment.Indexes of pulmonary function and blood gas analysis was compared before and 7 days after treatment.Results Before treatment,comparative statistical results of pulmonary function indexes and blood gas analysis indexes between two groups showed P>0.05.Intra group comparative statistical results of various pulmonary function indexes and blood gas analysis indexes between two groups before and after treatment showed P<0.05.After treatment,statistical results of pulmonary function indexes and blood gas analysis indexes between two groups showed P<0.05.Conclusion NIPPV combined with nalmefene can significantly improve respiratory function and hypoxia of elderly patients with COPD and severe type Ⅱ respiratory failure.
