NCPAP辅助治疗新生儿肺炎合并Ⅰ型呼吸衰竭的疗效及对血气分析指标的影响 被引量:1

摘要 目的分析经鼻部持续气道正压通气(nasal continuous positive airway pressure,NCPAP)辅助治疗对新生儿肺炎伴I型呼衰的疗效和血气指标的影响。方法选取2018年1月-2020年12月广东省雷州市人民医院收治的新生儿肺炎伴I型呼衰合计60例患儿进行研究,依据随机数表法划分成对照组(除基础治疗外加以头面部吸氧)和观察组(除基础治疗外加以NCPAP辅助治疗)各30例,对比两组总有效率、治疗前后呼吸(respiration rate,RR)、心率(heart rate,HR)、血氧饱和度(oxygen saturation,SPO_(2))、动脉血氧分压(partial pressure of oxygen,PaO_(2))、二氧化碳分压(partial pressure of carbon dioxide,PaCO_(2))、酸碱度值(pH value)、症状改善时间、住院时间以及并发症出现情况。结果观察组总有效率高出对照组(P<0.05)。治疗后,观察组RR、HR及PaCO_(2)水平低于对照组,SPO_(2)、PaO_(2)及pH值高于对照组,症状改善及住院时间短于对照组(P<0.05)。观察组并发症出现少于对照组(P<0.05)。结论NCPAP辅助治疗对新生儿肺炎伴I型呼衰疗效确切,能改善患儿的临床症状和血气分析指标,稳定其生命体征,缩短其住院时间,减少并发症出现,值得采用。 Objective To study the curative effects and blood gas analysis indicators for pneumonia neonates with type I respiratory failure using the adjuvant therapy of NCPAP(Nasal continuous positive airway pressure).Methods 60 pneumonia neonates with type I respiratory failure treated from January 2018 to December 2020 in Leizhou City People’s Hospital were investigated.According to random number table,neonates were divided into control group(n=30,basic treatment and oxygen uptake)and observation group(n=30,basic treatment and NCPAP).Group contrasts were performed through the total effective rate,respiratory rate(RR),heart rate(HR),oxygen saturation(SPO_(2)),partial pressure of oxygen(PaO_(2)),partial pressure of carbon dioxide(PaCO_(2)),pondus hydrogenii(pH value),symptom relief time,hospital stays and treatment complications.Results Total effective rate in the observation group was higher than control group(P<0.05);RR,HR and PaCO_(2) values in the observation group were lower than control group;SPO_(2),PaO_(2) and pH values were higher than control group;symptom relief time and hospital stays were shorter than control group(P<0.05);treatment complications in the observation group were fewer than control group(P<0.05).Conclusion NCPAP can improve the curative effect and blood gas analysis indicators,stabilize the vital signs,and reduce the clinical symptoms,hospital stays and treatment complications.It is worthy of application.
