摘要
目的对比分析温针灸联合拔罐或推拿对膝关节骨性关节炎(Knee osteoarthritis,KOA)患者的临床疗效。方法选取本院2017年10月-2020年7月收治的Kellgren和Lawrence分级为1~3级的KOA患者94例,将94例患者按随机数表法分为拔罐组和推拿组,每组47例。拔罐组接受温针灸联合拔罐疗法,推拿组接受温针灸联合推拿疗法。观察两组的临床疗效、疼痛评分和膝关节功能变化情况。结果拔罐组临床总有效率95.74%高于推拿组80.85%,差异有统计学意义(P<0.05),治疗前,两组患者的VAS评分和Womac关节炎指数比较,差异无统计学意义(P>0.05)。治疗后,两组VAS评分和Womac关节炎指数较治疗前均下降,且拔罐组下降更多(P<0.05)。两组患者治疗前hs-CRP和IL-6水平比较,差异无统计学意义(P>0.05)。治疗后,两组hs-CRP和IL-6水平均降低,且拔罐组低于推拿组,差异有统计学意义(P<0.05)。结论温针灸联合拔罐疗法对膝骨性关节炎的临床疗效更佳,临床可考虑推广应用。
Objective To compare and analyze the clinical effects of warm acupuncture combined with cupping or massage on patients with knee osteoarthritis(KOA).Methods 94 patients with KOA classified by Kellgren and Lawrence from October 2017 to July 2020 were selected.94 patients were randomly divided into cupping group and massage group,with 47 patients in each group.The cupping group received warm acupuncture combined with cupping therapy,and the massage group received warm acupuncture combined with massage therapy.The clinical efficacy,pain score and knee function were observed.Results The total clinical effective rate of cupping group was 95.74%higher than that of massage group 80.85%,the difference was statistically significant(P<0.05).Before treatment,there was no difference in VAS score and WOMAC arthritis index between the two groups(P>0.05).After treatment,VAS score and WOMAC arthritis index decreased in both groups,and decreased more in cupping group(P<0.05).There was no significant difference in the levels of hs-CRP and IL-6 between the two groups before treatment(P>0.05).After treatment,the levels of hs-CRP and IL-6 in the two groups were significantly lower than those in the massage group(P<0.05).Conclusion Warm acupuncture combined with cupping therapy has better clinical effect on knee osteoarthritis,which can be popularized in clinic.
