分析长期应用激素治疗对类风湿关节炎患者骨代谢及颈椎失稳发生率的影响

Effect Analysis of Long-Term Hormone Therapy on Bone Metabolism and Cervical Instability of Rheumatoid Arthritis Patients

目的观察长期采取激素治疗,对于类风湿关节炎患者骨代谢及颈椎失稳发生率的影响情况。方法选取2018年6月-2020年6月收治的80例类风湿性关节炎病患,划分成研究组、参照组,分别落实常规抗风湿药物治疗联合激素药物治疗、单纯常规抗风湿药物治疗方案,对比分析其28个关节疾病活动评分(DAS28)、类风湿因子(RF)水平、腰椎处骨密度(BMD)、颈椎失稳率、不良反应、疼痛程度评分(VAS)、炎性因子水平[血沉(ESP)、C反应蛋白(CRP)及类风湿因子(RF)水平]。结果治疗以后,对比类风湿关节炎患者病情评分、类风湿因子水平,研究组显著的低于参照组,P<0.05;对比颈椎失稳发生率,研究组(5.00%)显著的低于参照组(22.50%),P<0.05;与参照组比较,研究组VAS评分、ESP、CRP、RF水平显著低于参照组,P<0.05。结论类风湿关节炎患者长期应用激素治疗,能够促进其病情积极改善,不会影响骨代谢,减少颈椎失稳率,同时还能减轻患者的疼痛感,改善其炎症反应。

Objective To observe effect of long-term hormone treatment on bone metabolism and cervical instability of patients with rheumatoid arthritis.Methods The paper chose 80 patients with rheumatoid arthritis from June 2018 to June 2020,and divided them into study group and reference group,treated with routine anti rheumatic drugs combined with hormone drugs,and routine anti rheumatic drugs only.28 joint disease activity scores(DAS28),rheumatoid factor(RF),lumbar bone mineral density(BMD),cervical instability rate,adverse reactions Pain scores(VAS),inflammatory factor level[erythrocyte sedimentation rate(ESP),C-reactive protein(CRP)and rheumatoid factor(RF)]were compared between two groups.Results After treatment,condition scores of patients with rheumatoid arthritis and rheumatoid factor level in study group were significantly lower than reference group(P<0.05);incidence of cervical instability in study group(5.00%)was significantly lower than reference group(22.50%),P<0.05;VAS scores,ESP,CRP and RF levels in study group were significantly lower than reference group,(P<0.05).Conclusion Long-term application of hormone therapy can promote positive condition improvement of patients with rheumatoid arthritis,may not affect bone metabolism seriously,reduce incidence of cervical instability and pains,improve inflammatory response of patients.
