基于信息化平台构建全院血糖管理模式的临床实践

Clinical practice of establishing hospital blood glucose management mode based on information platform

目的该研究拟构建信息化的全院血糖管理模式以提高糖尿病患者的血糖管理效率。方法利用信息技术手段,将血糖监测系统整合加入已有的医院信息系统(HIS),构建全院血糖管理系统,同时组建全院血糖管理团队,评估临床干预效果。结果基于信息化平台下的全院血糖管理模式能够有效改善非内分泌科糖尿病患者的血糖水平,并且在信息化全院血糖管理模式下内分泌科会诊后,非内分泌科糖尿病患者的总体血糖水平明显优于传统的会诊模式。结论信息化的全院血糖管理模式能够有效提高全院非内分泌科糖尿病患者的血糖管理效率。

【Objective】To construct an informationized whole-hospital blood glucose management model to improve the efficiency of blood glucose management in diabetic patients.【Methods】In this research,information technology was used to optimize the blood glucose monitoring in the hospital,and the existing hospital information system(HIS)was integrated to build the blood glucose management system of the hospital.Meanwhile,the blood glucose management team of the hospital was established.Informationized and personalized whole-hospital blood glucose management was put into clinical practice to evaluate the effect of intervention.【Results】The blood glucose management team of the hospital includes endocrinologists,diabetes care team,nutritionists and clinical pharmacists.The whole-hospital blood glucose management model based on the information-based platform can effectively improve the blood glucose level of non-endocrine diabetic patients.【Conclusion】The informationized blood glucose management model of the whole hospital can effectively improve the blood glucose management efficiency of non endocrinology diabetic patients.
