生物标志物ADH1C、PCK1、MARCO在急性阑尾炎诊断中的作用 被引量:1

摘要 目的探讨急性阑尾炎的潜在生物标志物酒精脱氢酶1C(ADH1C)、磷酸烯醇式丙酮酸羧激酶1(PCK1)、胶原结构巨噬细胞受体(MARCO)在急性阑尾炎的诊断中的作用。方法采用GSE9579阵列用于差异化鉴定急性阑尾炎相关标志物,选取2020年1月至2021年6月在泌阳白云山中西医结合医院急诊科住院的80例腹痛患者作为研究对象。其中最终确诊为急性阑尾炎的患者46例为阑尾组,其他原因腹痛的患者34例为对照组。通过酶联免疫吸附试验(ELISA)分别检测不同组患者的血清ADH1C、PCK1、MARCO水平,比较两组患者血清ADH1C、PCK1、MARCO水平异常率及血清学水平。利用曲线下面积(AUC)值检验ADH1C、PCK1、MARCO的诊断性能。结果从GSE25765数据集中,进行阑尾组与对照组的差异性基因表达,发现ADH1C、PCK1显著下调,而MARCO显著上调。KEGG富集分析发现这些差异基因参与P450-药物代谢、酪氨酸代谢、丙酮酸代谢、类固醇激素生物合成及PPAR信号通路等生物学功能。与对照组相比,阑尾组患者血清中ADH1C、PCK1含量显著均较低,MARCO含量显著升高(P<0.05)。血清ADH1C、PCK1、MARCO水平对急性阑尾炎分别具有良好的诊断效能(AUC值>0.07)。结论ADH1C、PCK1、MARCO在急性阑尾炎患者中变化显著,对阑尾炎的临床诊断具有一定指导意义。 【Objective】To explore the role of potential biomarkers of acute appendicitis,alcohol dehydrogenase 1 C(ADH 1 C),phosphoenolpyruvate carboxykinase 1(PCK 1)and collagen macrophage receptor(MARCO)in the diagnosis of acute appendicitis.【Methods】GSE 9579 array was used to differentiate and identify markers related to acute appendicitis.Forty patients with acute appendicitis in the emergency department of Baiyun Mountain Hospital of Integrated Traditional Chinese and Western Medicine from January 2020 to June 2021 were selected as the appendiceal group,and 40 patients with non-inflammatory abdominal pain in the emergency department of our hospital at the same time were selected as the control group.The levels of serum ADH 1 C,PCK 1 and MARCO in different groups were detected by ELISA,and the abnormal rates and serological levels of serum ADH 1 C,PCK 1 and MARCO were compared between the two groups.The AUC value was used to test the diagnostic performance of ADH 1 C,PCK 1 and MARCO.【Results】From the GSE 25765 data set,the differential gene expression between the appendix group and the control group was found that ADH 1 C and PCK 1 were significantly down-regulated,while MARCO was significantly up-regulated.KEGG enrichment analysis showed that these differential genes were involved in P 450-drug metabolism,tyrosine metabolism,pyruvate metabolism,steroid biosynthesis and PPAR signal pathway and other biological functions.Compared with the control group,the contents of ADH 1 C and PCK 1 in the appendix group were significantly lower and the MARCO content was significantly higher in the appendix group(P<0.05).The levels of serum ADH 1 C,PCK 1 and MARCO have good diagnostic efficacy in acute appendicitis(AUC>0.07).【Conclusion】ADH 1 C,PCK 1 and MARCO change significantly in patients with acute appendicitis,which has a certain guiding significance for the clinical diagnosis of appendicitis.
