抗生素降阶梯疗法治疗新生儿感染性肺炎患儿的效果及其对血清学感染指标水平的影响价值 被引量:2

摘要 目的探讨新生儿感染性肺炎患儿应用抗生素降阶梯疗法的效果及其对血清学感染指标水平的影响价值。方法选取2019年8月至2021年3月期间平顶山市妇幼保健院收治的97例新生儿感染性肺炎患儿作为研究对象,以抽签的方法进行分组。对照组48例给予常规抗生素治疗,观察组49例给予抗生素降阶梯疗法,比较两组患儿临床疗效、症状控制情况、血清学感染指标、不良反应。结果观察组患儿总有效率高于对照组(P<0.05);观察组患儿体温恢复时间、肺啰音消失时间、呼吸平稳时间、住院时间均低于对照组(P<0.05);观察组患儿血清降钙素原(PCT)、C反应蛋白(CRP)水平、白细胞(WBC)计数均低于对照组(P<0.05);两组患儿腹泻、呕吐、鹅口疮发生率比较,差异无统计学意义(P>0.05)。结论抗生素降阶梯疗法治疗新生儿感染性肺炎患儿效果确切,能够有效控制感染情况,减轻临床症状,缩短住院时间,改善CRP、WBC、PCT水平,具备较高的临床推广应用价值。 【Objective】To investigate the effect of antibiotics step-down therapy on serological infection indexes in neonates with infective pneumonia.【Methods】A total of 97 children with neonatal infectious pneumonia admitted to our hospital from August 2019 to March 2021 were selected as the research objects and divided into groups by drawing lots.Forty-eight cases in the control group were treated with conventional antibiotics,and 49 cases in the observation group were treated with antibiotics step down therapy.Clinical efficacy,symptom control,serological infection indicators and adverse reactions were compared between the two groups.【Results】The total effective rate in observation group was higher than that in control group(P<0.05).The temperature recovery time,lung rales disappearance time,stable breathing time and hospital stay time in observation group were lower than those in control group(P<0.05).The levels of serum procalcitonin(PCT),C-reactive protein(CRP)and white blood cell(WBC)in observation group were lower than those in control group(P<0.05).The incidence of diarrhea,vomiting and thrush was compared between the two groups with no statistically siginificant(P>0.05).【Conclusion】Antibiotic step-down therapy is effective in the treatment of neonatal infectious pneumonia,which can effectively control infection,alleviate clinical symptoms,shorten hospital stay,improve CRP,WBC,PCT levels,and is worthy of clinical application.
