吞咽功能训练在口腔癌行游离组织瓣移植修复术后患者中的应用效果

Effect of swallowing function training on patients with oral cancer after free tissue flap transplantation

目的探究吞咽功能训练在游离组织皮瓣移植修复口腔癌术后组织缺损患者中的应用效果。方法选择某三甲医院头颈肿瘤外科于2020年8月—2020年12月收治的80例口腔癌行游离组织皮瓣移植术后患者作为研究对象,将患者随机分为实验组和对照组,每组各40例。对照组患者采用常规治疗护理与吞咽功能评定。实验组在对照组护理的基础上对患者进行术后吞咽功能训练。观察并比较两组患者吞咽功能障碍程度、平均住院时间、留置胃管时间。结果吞咽功能训练后,实验组改善吞咽功能障碍有效率明显高于对照组,留置胃管时间明显低于对照组,差异均有统计学意义(P<0.05);两组患者的平均住院时间无统计学意义(P>0.05)。结论早期吞咽功能评定联合吞咽功能训练能有效改善口腔癌行游离组织皮瓣移植术后患者的吞咽功能障碍,进而减少留置胃管时间以及并发症的发生。

Objective To explore the effect of swallowing function training on patients with oral cancer after free tissue flap transplantation.Methods A total of 80 patients with oral cancer who underwent free tissue flap transplantation from August 2020 to December 2020 in the head and neck tumor surgery department of a third grade A hospital were selected as the research objects.The patients were randomly divided into experimental group and control group,with 40 cases in each group.The control group received conventional treatment nursing and swallowing function evaluation.The experimental group in the control group nursing on the basis of patients with postoperative swallowing function training.The degree of swallowing dysfunction,average length of hospital stay and indwelling time of gastric tube were observed and compared between the two groups.Results The effective rate of improving swallowing dysfunction in the experimental group was significantly higher than that in the control group,the indwelling time of gastric tube in the experimental group was significantly lower than that in the control group(P<0.05),and there was no significant difference in the average length of stay between the two groups(P>0.05).Conclusion Swallowing function assessment combined with swallowing function training can effectively improve swallowing dysfunction and reduce the time of gastric tube indwelling for oral cancer after free flap transplantation.
