腹腔镜广泛子宫切除联合淋巴结清扫术在宫颈癌治疗中的效果观察

Observation on effect of laparoscopic radical hysterectomy combined with lymph node dissection in the treatment of cervical cancer

目的对比开腹与腹腔镜广泛子宫切除联合淋巴结清扫术在宫颈癌治疗中的效果。方法116例宫颈癌患者,按照手术方法的不同分为对照组与观察组,各58例。对照组行常规开腹广泛子宫切除联合淋巴结清扫术,观察组行腹腔镜广泛子宫切除联合淋巴结清扫术。对比两组患者术后并发症发生情况以及手术相关指标。结果观察组术后并发症发生率为5.17%,低于对照组的20.69%,差异具有统计学意义(P<0.05)。观察组术中出血量(278.24±45.26)ml少于对照组的(385.83±44.32)ml,术后排气时间(2.48±1.95)d、住院时间(9.97±2.48)d短于对照组的(5.15±1.01)、(15.78±2.72)d,差异具有统计学意义(P<0.05);两组淋巴结清扫数比较,差异无统计学意义(P>0.05)。结论腹腔镜广泛子宫切除联合淋巴结清扫术治疗宫颈癌与开腹手术比较,具有并发症发生率低、术中出血量少、淋巴结清扫彻底等优势,适宜作为宫颈癌的常规术式。

Objective To compare the effect of laparoscopic radical hysterectomy combined with lymph node dissection in the treatment of cervical cancer.Methods A total of 116 patients with cervical cancer were divided into control group and observation group according to different surgical methods,with 58 cases in each group.The control group underwent conventional open hysterectomy combined with lymph node dissection,while the observation group underwent laparoscopic wide hysterectomy combined with lymph node dissection.The postoperative complication rate and surgery-related indicators were compared between the two groups.Results The postoperative complication rate in the observation group was 5.17%,which was lower than 20.69%in the control group,and the difference was statistically significant(P<0.05).In the observation group,the intraoperative blood loss(278.24±45.26)ml was less than(385.83±44.32)ml in the control group;the postoperative exhaust time(2.48±1.95)d and hospitalization time(9.97±2.48)d were shorter than(5.15±1.01)and(15.78±2.72)d in the control group;all the differences were statistically significant(P<0.05).There was no statistically significant difference in the number of lymph node dissections between the two groups(P>0.05).Conclusion Compared with open surgery,laparoscopic radical hysterectomy combined with lymph node dissection has the advantages of low complication rate,less intraoperative bleeding and thorough lymph node dissection,and is suitable as a conventional procedure for cervical cancer.
