摘要
目的分析心脏超声在高血压左室肥厚伴左心衰竭诊断中的应用价值。方法选取99例高血压左室肥厚伴左心衰竭患者作为研究组,另选取同期接受体检的健康志愿者100例作为对照组。两组均接受心脏彩色多普勒超声检查。比较两组左心结构检测指标[左心房内径(LAD)、左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)、左心室后壁舒张末期厚度(LVPWT)、室间隔厚度(IVST)]以及左心功能检测指标[左心室射血分数(LVEF)、二尖瓣血流频谱E峰与二尖瓣环运动速度Ea比值(E/Ea)]。结果研究组患者的LAD(46.23±5.22)mm、LVESD(32.46±1.51)mm、LVEDD(53.09±5.58)mm、LVPWT(8.64±0.36)mm、IVST(9.13±1.35)mm均高于对照组的(32.82±2.95)、(27.11±1.19)、(44.38±4.02)、(8.35±0.27)、(8.22±1.08)mm,差异均具有统计学意义(P<0.05)。研究组患者LVEF(54.85±14.09)%低于对照组的(66.87±19.43)%,E/Ea(14.55±5.23)高于对照组的(8.03±1.32),差异具有统计学意义(P<0.05)。结论采用心脏超声对高血压左室肥厚伴左心衰竭患者进行诊断,可以有效判断患者的心脏结构、功能的异常改变情况,评估患者的病情进展,为患者的治疗提供依据。
Objective To analyze the application value of echocardiography in the diagnosis of hypertensive left ventricular hypertrophy with left heart failure.Methods 99 patients with hypertensive left ventricular hypertrophy and left heart failure were selected as the research group,and 100 healthy volunteers who received physical examination during the same period were selected as the control group.Both groups underwent echocardiography.Both groups were compared in terms of left ventricular structural measurement indicators[left atrial diameter(LAD),left ventricular end-systolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD),left ventricular posterior wall thickness(LVPWT),inter-ventricular septal thickness(IVST)]and left ventricular function test indicators[left ventricular ejection fraction(LVEF),ratio of mitral valve blood flow spectrum E peak and mitral valve ring motion velocity Ea(E/Ea)].Results LAD(46.23±5.22)mm,LVESD(32.46±1.51)mm,LVEDD(53.09±5.58)mm,LVPWT(8.64±0.36)mm and IVST(9.13±1.35)mm in the research group were significantly higher than(32.82±2.95),(27.11±1.19),(44.38±4.02),(8.35±0.27),and(8.22±1.08)mm in the control group,and the differences were statistically significant(P<0.05).The LVEF(54.85±14.09)%in the research group was lower than(66.87±19.43)%in the control group,and the E/Ea(14.55±5.23)was higher than(8.03±1.32)in the control group.All the differences were statistically significant(P<0.05).Conclusion The use of echocardiography in the diagnosis of hypertensive left ventricular hypertrophy with left heart failure can effectively determine the abnormal changes of the patient's cardiac structure and function,evaluate the patient's disease progression,and provide a basis for the patient's treatment.
