低频脉冲电刺激疗法联合健侧肢体运动诱导患侧肢体运动治疗卒中后偏瘫患者的效果

摘要 目的:观察低频脉冲电刺激疗法联合健侧肢体运动诱导患侧肢体运动治疗卒中后偏瘫(HAS)患者的效果。方法:选取126例HAS患者为研究对象,按照随机数字表法分为对照组和观察组各63例。两组均接受常规治疗,在此基础上,对照组予以健侧肢体运动诱导患侧肢体运动,观察组在对照组基础上予以低频脉冲电刺激疗法。比较两组治疗前后肢体运动功能[简易Fugl-Meyer运动功能量表(FMA)]评分、血流动力学指标[收缩期血流速度(Vs)、阻力指数(RI)、平均血流速度(Vm)]水平、肢体平衡能力[Berg平衡量表(BBS)]评分、步行功能[功能性步行量表(FAC)]评分和脑卒中影响量表(SIS)评分。结果:治疗后,观察组上肢、下肢和总分等FMA评分均明显高于对照组,差异有统计学意义(P<0.05);观察组Vs、Vm水平均高于对照组,RI水平低于对照组,差异有统计学意义(P<0.05);观察组BBS、FAC、SIS评分均高于对照组,差异有统计学意义(P<0.05)。结论:低频脉冲电刺激疗法联合健侧肢体运动诱导患侧肢体运动应用于HAS患者,可提高FMA、BBS、FAC、SIS评分,改善血流动力学指标水平,效果优于单纯健侧肢体运动诱导患侧肢体运动。 Objective:To observe effects of low-frequency pulse electrical stimulation therapy combined with healthy limb movement-induced affected limb movement on patients with hemiplegia after stroke(HAS).Methods:126 patients with HAS were selected as the research objects and were divided into control group and observation group according to the random number table method,63 cases in each group.Both groups received conventional treatment.On this basis,the control group was given the healthy limb movement-induced affected limb movement,while the observation group used the low-frequency pulse electrical stimulation therapy on the basis of that of the control group.The limb motor function[simple Fugl-Meyer assessment(FMA)]score,the hemodynamic index levels[systolic blood flow velocity(Vs),resistance index(RI),mean blood flow velocity(Vm)],the limb balance[Berg balance scale(BBS)]score,the walking function[functional ambulation category scale(FAC)]score,and the stroke impact scale(SIS)score were compared between the two groups before and after the treatment.Results:After the treatment,the FMA scores of upper extremity and lower extremity as well as the total score in the observation group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of Vs and Vm in the observation group were higher than those in the control group,the level of RI was lower than that in the control group,and the differences were statistically significant(P<0.05).Further,the BBS,FAC and SIS scores of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).Conclusions:Low-frequency pulse electrical stimulation therapy combined with healthy limb movement-induced affected limb movement on the patients with HAS can improve the FMA,BBS,FAC,SIS scores and improve the hemodynamic indicators.Moreover,it is superior to single healthy limb movement-induced affected limb movement.
