血清TK1结合肿瘤标记物评估乳腺癌新辅助化疗疗效的价值观察 被引量:7

Value of serum TK1 combined with tumor markers in curative effect evaluation of neoadjuvant chemotherapy for breast cancer

摘要 目的:分析血清胸苷激酶1(TK1)结合肿瘤标记物对乳腺癌新辅助化疗疗效的评估价值。方法:选取150例乳腺癌患者为研究对象,按照新辅助化疗临床疗效分为有效组(n=96)与无效组(n=54)。比较两组患者化疗前后血清TK1、肿瘤标记物[糖类抗原15-3(CA15-3)、糖类抗原125(CA125)、癌胚抗原(CEA)、肿瘤异常蛋白(TAP)]水平;Logistic回归模型分析上述指标与疗效的关系;受试者工作特征(ROC)曲线分析TK1结合肿瘤标记物对疗效评估的价值。结果:化疗后,两组患者血清TK1、CA15-3及TAP水平比较,差异有统计学意义(P<0.05);回归分析显示,化疗后血清TK1、CA15-3、TAP水平与乳腺癌新辅助化疗疗效有关(P<0.05);ROC曲线分析显示,化疗后血清TK1、CA15-3、TAP水平及联合评估化疗疗效曲线下面积(AUC)分别为0.867、0.766、0.685、0.888,联合评估AUC最大,敏感度与特异度分别为90.7%、77.1%。结论:乳腺癌患者新辅助化疗后血清TK1、CA15-3、TAP水平与化疗疗效有关,三者联合检测对化疗疗效具有较高评估价值。 Objective:To analyze the value of serum thymidine kinase 1(TK1)combined with tumor markers in curative effect evaluation of neoadjuvant chemotherapy for breast cancer.Methods:150 breast cancer patients treated with neoadjuvant chemotherapy were divided into effective group(n=96)and ineffective group(n=54)according to the clinical effect.Serum TK1 and tumor markers[carbohydrate antigen 15-3(CA15-3),carbohydrate antigen 125(CA125),carcinoembryonic antigen(CEA),tumor abnormal protein(TAP)]were detected before and after chemotherapy.Logistic regression model was used to analyze the relationship between indicators and curative effect.The receiver operating characteristic(ROC)curve was used to analyze the value of TK1 combined with tumor markers in curative effect evaluation.Results:There were statistically significant differences in TK1,CA15-3 and TAP between the two groups after chemotherapy(P<0.05).Logistic regression analysis showed that TK1,CA15-3 and TAP after chemotherapy were related to the curative effect of neoadjuvant chemotherapy for breast cancer(P<0.05).ROC analysis showed that the area under the curve(AUC)values of TK1,CA15-3,TAP alone and in combination to evaluate the curative effect of chemotherapy were 0.867,0.766,0.685,and 0.888,respectively.The AUC of combined evaluation was the largest,and the sensitivity and specificity were 90.7%and 77.1%.Conclusion:Serum TK1,CA15-3 and TAP in breast cancer patients after neoadjuvant chemotherapy are related to the curative effect of chemotherapy.Combination of the three is of high value in curative effect evaluation of chemotherapy.
