基于数据挖掘的中药治疗肝性脑病用药规律分析

目的基于数据挖掘技术探讨中药治疗肝性脑病(HE)的用药规律,以期为临床治疗HE提供新思路。方法检索自建库以来至2021-06-01收录于中国知识基础设施工程(CNKI)、万方数据知识服务平台、重庆维普资讯有限公司、中国生物医学文献数据库(CBM)、PubMed、荷兰医学文献数据库和Cochrane图书馆中中药治疗HE的临床研究文献的条目,记录文献中临床试验类型、治疗组使用中药方剂名称及具体组成药物。使用EXCEL 2019、SPSS 22.0及SPSS Modeler 18.1进行统计学分析,利用Gephi v 0.9.2进行可视化分析。结果共纳入文献52篇,拆分得到中药处方55条,共涉及中药106味。药味、药性以苦寒为主,归经以肝、胃、脾经为主,功效以清热、攻下、理气为主,高频药物中大黄、石菖蒲、厚朴使用频次位列前3。通过聚类分析得到常用药对枳实-赤芍等8个、核心组合茵陈-虎杖-栀子-牡蛎等4组,通过关联分析得到具有较强关联性的药物组合黄芩-黄连等5组,通过高频中药可视化分析得到以大黄、郁金、石菖蒲为主的核心药物群。结论中药治疗HE具有一定规律性,总体以清热祛湿、理气开郁、开窍醒神为主,重视肝、脾、胃同治。

Objective To explore the data mining in medication rule of China herb for hepatic encephalopathy(HE),aiming to provide novel theory for the clinical treatment.Methods Until to June 1,2021,we collected literature entries regarding China herb for HE which published in China national knowledge internet(CNKI),Wanfang database,VIP database,Chinese Biomedical Database(CBM),PubMed,Dutch Medical literature database,Cochrane library,and we asessed test types,prescription name and drug composition,using Microsoft Excel software(version 2019),Statistical Product and Service Solutions(SPSS)softare(version 22.0)and SPSS Modeler(version 18.1)for statistical analysis and Gephi software(version 0.9.2)for visual analysis.Results There were 52 literatures,55 China herb prescriptions,106 Chinese herbs in this study.Bitterness and coldness were the main manifestations of drug taste and drug properties,liver,stomach and spleen were the main parts of drug action,and Qingre,Qvxie and Fuzheng,and Liqi were the main features of drug efficacy.Dahuang,Shichangpu and Houpo occupied the top three high-frequency drugs.Cluster analysis evidenced 8 commonly-used couplet medicines(Zhishi-Chishao)and 4 core combinations(Yinchen-Huzhang-Zhizi-Muli),the association analysis showed 5 drug combinations with strong correlation(Huangqin-Huanglian),the visual analysis of high-frequency China herbs indicated core drug group mainly including Dahuang,Yujin,Shichangpu.Conclusion The regularity of Chinese herb for HE mainly characterised by Qingre and Qvshi,Liqi and Kaiyu,Kaiqiao and Xingshen,with simultaneous treatment of liver,spleen and stomach.
