难治性肺炎支原体肺炎患儿外周血PCT、DD及IL-17水平的检测意义

Significance of detecting peripheral blood procalcitonin,D-Dimer,and Interleukin-17 levels in children with refractory mycoplasma pneumoniae pneumonia

目的探讨外周血降钙素原(PCT)、D-二聚体(DD)、白介素-17(IL-17)在难治性肺炎支原体肺炎(RMPP)患儿外周血中的表达水平及与预后的相关性。方法选取南阳市某医院2018年1月—2021年1月收治的96例RMPP患儿为研究对象,依据其不同临床结局分为进展组(28例)及对照组(68例),所有入组患儿均于入院第2天检测外周血PCT、DD及IL-17水平。比较2组患儿外周血PCT、DD及IL-17表达水平,采用Spearman相关性分析其与患儿预后的相关性,并绘制受试者工作特征(ROC)曲线评定其对RMPP疾病进展的预测效能。结果进展组患儿外周血PCT、DD及IL-17水平均高于对照组,差异均有统计学意义(P<0.05)。外周血PCT、DD及IL-17水平均与RMPP患儿预后均呈显著正相关(P<0.05)。外周血PCT、DD及IL-17水平预测RMPP疾病进展的曲线下面积(AUC)分别为0.657、0.678、0.712,截断值分别为0.89 ng/L、6.72 mg/L、2.59 ng/L,三者联合预测AUC为0.791。结论外周血PCT、DD及IL-17水平均与RMPP患儿预后密切相关,对评估患儿病情严重程度具有良好预测价值。

Objective To investigate the expression of procalcitonin(PCT),D-dimer(DD)and interleukin-17(IL-17)in peripheral blood of children with refractory mycoplasma pneumoniae pneumonia(RMPP)and their correlation with prognosis.Methods A total of 96 children with RMPP admitted to a hospital in Nanyang from January 2018 to January 2021 were selected as the subjects and assigned to the progression group(n=28)and the control group(n=68)following their different clinical outcomes.The peripheral blood PCT,DD and IL-17 levels were measured on the second day after admission in all children.The expression levels of peripheral blood PCT,DD,and IL-17 of the two groups were compared.Spearman correlation was used to analyze their correlation with the prognoses of the children,and receiver operating characteristic(ROC)curves were plotted to assess their predictive efficacy for RMPP disease progression.Results The levels of peripheral blood PCT,DD and IL-17 in the progression group were higher than those of the control group,and the differences were statistically significant(P<0.05).There were significant positive correlations between peripheral blood PCT,DD and IL-17 levels and the prognosis of children with RMPP(P<0.05).The Area Under the Curve(AUC)of peripheral blood PCT,DD,and IL-17 levels in predicting RMPP disease progression was divided into 0.657,0.678,and 0.712,with cutoff values of 0.89 ng/L,6.72 mg/L,and 2.59 ng/L,respectively,and the AUC of the combination of the three was 0.791.Conclusion The peripheral blood PCT,DD and IL-17 levels are closely related to the prognosis of children with RMPP,which have a good predictive value for evaluating the severity of the disease.
