某院门急诊孟鲁司特钠片不合理使用现状分析

Status quo analysis on the irrational use of montelukast sodium tablets in the outpatient emergency department of a hospital

目的调查某院门急诊孟鲁司特钠片不合理使用情况,分析其使用原因,改正临床孟鲁司特钠片不合理使用现象。方法通过医院HIS系统抽取2020年8月—2021年7月开具孟鲁司特钠片的2106张电子处方,对不合理使用孟鲁司特钠片处方存在的问题以及开具药品科室分布情況等进行统计分析。结果抽取的2106张处方中,不合理处方1121张,占处方总数的53.23%,不合理用药以超说明书适应症用药为主,其中只存在超说明书适应症用药的处方1096张,占不合理处方的97.77%,主要涉及科室为呼吸内科门诊、社区全科医学科门诊、神经内科门诊等。结论我院孟鲁司特钠片不合理使用现象以超说明书适应症用药为主,医院应加强医生合理用药知识培训,药师应有效地利用现代化工具加强对合理用药的监管,纠正临床不合理的超说明书适应症用药现象。

Objective To investigate the irrational use of montelukast sodium tablets in the outpatient emergency department of a hospital,analyze the causes of its use,and correct the irrational use of montelukast sodium tablets in clinical practice.Methods A total of 2106 electronic prescriptions of montelukast sodium tablets prescribed from August 2020 to July 2021 were selected by the Hospital Information System(HIS)system,and the existing problems of irrational use of montelukast sodium tablets and the distribution of drug-prescribing departments were statistically analyzed.Results Of the 2106 prescriptions selected,1121(53.23%)were irrational prescriptions,mostly characterized by off-label drug use,of which 1096(97.77%)were off-label drug use,mainly prescribed by the respiratory medicine clinics,community general medicine clinics,and neurology clinics.Conclusion The irrational use of montelukast sodium tablets in our hospital is mainly off-label drug use.The hospital should strengthen the training of doctors on rational drug use.Pharmacists should effectively use modern tools to strengthen the supervision of rational drug use and correct the irrational off-label drug use in clinical practice.
