系统化整体护理在腹腔镜胃癌根治术围手术期护理中的应用

Application of systematic holistic nursing in perioperative nursing of laparoscopic radical gastrectomy for gastric cancer

目的探讨系统化整体护理在腹腔镜胃癌(GC)根治术围手术期护理中的应用效果。方法选取2018年1月—2020年1月于南阳市某医院接受腹腔镜GC根治术治疗的81例GC患者为研究对象,采用随机数字表法分为研究组(41例)与对照组(40例),给予研究组患者系统化整体护理干预,给予对照组患者常规护理干预,2组患者均持续干预3周。比较2组患者干预前后汉密尔顿抑郁量表(HAMD)评分、日常生活能力量表(ADL)评分、汉密尔顿焦虑量表(HAMA)评分,比较2组患者护理满意度及护理期间并发症发生情况。结果干预前,2组患者HAMA、HAMD评分比较,差异均无统计学意义(P>0.05)。干预后,2组患者HAMA、HAMD评分均低于干预前,且研究组低于对照组,差异均有统计学意义(P<0.05)。干预前,2组患者进阶能力、基本能力评分、ADL总评分比较,差异均无统计学意义(P>0.05)。干预后,2组患者进阶能力、基本能力评分、ADL总评分均高于干预前,且研究组高于对照组,差异均有统计学意义(P<0.05)。研究组患者护理满意度为97.56%,高于对照组的82.50%,差异有统计学意义(P<0.05)。研究组患者护理期间并发症发生率为4.88%,低于对照组的20.00%,差异有统计学意义(P<0.05)。结论系统化整体护理在GC患者腹腔镜胃癌根治术围手术期中的应用效果确切,可有效减轻患者围手术期负性情绪,改善患者生活质量并降低围手术期并发症发生风险,具有广阔应用前景。

Objective To explore the effect of systematic holistic nursing in perioperative nursing of laparoscopic radical gastrectomy for gastric cancer(GC).Methods A total of 81 patients with GC treated with laparoscopic radical gastrectomy in a hospital in Nanyang City from January 2018 to January 2020 were selected as subjects,and were assigned to a study group(n=41)and a control group(n=41)using the random number table method.The patients in the study group received systematic holistic nursing interventions during the perioperative period and the patients in the control group received routine nursing interventions during the perioperative period.The patients in both groups received continuous intervention for 3 weeks.The Hamilton Depression Scale(HAMD)scores,Activity of Daily Living(ADL)Scale scores and Hamilton Anxiety Scale(HAMA)scores were compared before and after the intervention,and the patients’satisfaction with nursing and complications during nursing were compared.Results Before the intervention,there was no statistically significant difference in the HAMA and HAMD scores of the two groups(P>0.05).After the intervention,the HAMA and HAMD scores of the 2 groups were lower than those before the intervention,and the study group was lower than the control group,with statistically significant differences(P<0.05).Before the intervention,there was no significant difference in advanced ability,basic ability and ADL total scores between the two groups(P>0.05).After the intervention,the advanced ability,basic ability and ADL total scores of the two groups were higher than those before the intervention,and the study group was higher than the control group,with statistical significance(P<0.05).The nursing satisfaction of patients in the study group was 97.56%,which was higher than that in the control group(82.50%),and the difference was statistically significant(P<0.05).The incidence of complications during nursing in the study group was 4.88%,which was lower than that in the control group(20.00%),and the difference was statistically significant(P<0.05).Conclusion The effect of systematic holistic nursing in the perioperative period of laparoscopic radical gastrectomy for patients with GC is exact,which can effectively reduce the negative emotions,improve the quality of life and reduce the risk of complications of patients in the perioperative period.It thus has broad application prospects.
