肋间神经阻滞联合早期非甾体类抗生素用于单孔胸腔镜肺癌术后镇痛效果观察 被引量:1

摘要 目的:探讨肋间神经阻滞(INB)联合术后早期使用非甾体类抗炎药在单孔胸腔镜肺部手术患者中的应用效果。方法:选取2020年6月—2021年10月收治的60例行单孔胸腔镜下肺部手术治疗的肺癌患者为研究对象,按照随机数字表法分为对照组(30例)与观察组(30例)。对照组采用常规护理管理,观察组在对照组基础上应用超前镇痛和INB,术后早期使用非甾体类抗炎药。比较两组镇痛满意度、中度疼痛以上发生率、并发症发生率、早期下床时间以及直接经济负担。结果:观察组镇痛满意度(83.33%)明显高于对照组(60.00%),差异有统计学意义(P<0.05);观察组中度以上疼痛发生率(63.33%)明显低于对照组(86.67%),差异有统计学意义(P<0.05);观察组并发症总发生率(10.00%)明显低于对照组(33.33%),差异有统计学意义(P<0.05);观察组早期下床活动时间明显短于对照组,且观察组直接经济负担低于对照组,差异均有统计学意义(P<0.05)。结论:INB联合术后早期使用非甾体类抗炎药应用于单孔胸腔镜肺部手术患者中,可提升镇痛效果,减轻患者疼痛症状,加快康复速度。 Objective:To investigate the effect of intercostal nerve blockade(INB)combined with early postoperative non-steroidal anti-inflammatory drugs in patients with single-port thoracoscopic lung surgery.Methods:A total of 60 patients with lung cancer who underwent single-port thoracoscopic lung surgery from June 2020 to October 2021 were selected as the research subjects,according to random number table method was divided into control group(30 cases)and observation group(30 cases)and control group with routine nursing management,group on the basis of the application of advanced analgesia and INB,non-steroidal anti-inflammatory drugs were used early after surgery.Pain satisfaction,incidence of moderate pain,incidence of complications,time to get out of bed early and direct economic burden were compared between the two groups.Results:The pain satisfaction in the observation group(83.33%)was significantly higher than that in the control group(60.00%)(P<0.05).The incidence of moderate pain in observation group(63.33%)was significantly lower than that in control group(86.67%),and the difference was statistically significant(P<0.05).The total incidence of complications in the observation group(10.00%)was significantly lower than that in the control group(33.33%)(P<0.05).The early ambulation time of the observation group was significantly shorter than that of the control group,and the direct economic burden of the observation group was lower than that of the control group(P<0.05).Conclusion:The application of INB combined with the early postoperative use of nsaids in single-port thoracoscopic lung surgery can improve the analgesic effect,relieve the pain symptoms of patients,and speed up the recovery rate.
