腹腔镜结合促性腺激素释放激素激动剂治疗子宫内膜异位症伴不孕症的临床研究

Clinical study of laparoscopy combined with gonadotropin-releasing hormone agonist in the treatment of endometriosis with infertility

目的探究腹腔镜结合促性腺激素释放激素激动剂治疗子宫内膜异位症伴不孕症的临床疗效。方法选取2018年2月至2019年7月本院收治的140例子宫内膜异位症伴不孕症患者作为研究对象,随机分为对照组和观察组,每组70例。对照组行腹腔镜治疗,观察组在对照组基础上结合促性腺激素释放激素激动剂治疗,比较两组激素水平、临床疗效及妊娠情况。结果治疗前,两组促卵泡素(FSH)、血清雌二醇(E2)、促黄体生成素(LH)水平比较差异无统计学意义;治疗后,观察组FSH、E2及LH水平均低于对照组,差异有统计学意义(P<0.05);观察组治疗总有效率为94.29%,高于对照组的67.14%,差异有统计学意义(P<0.05);观察组妊娠率和周期排卵率均高于对照组,差异有统计学意义(P<0.05);两组流产率比较差异无统计学意义。结论腹腔镜结合促性腺激素释放激素激动剂治疗子宫内膜异位症伴不孕症效果显著,可有效改善患者激素水平,提高妊娠率,值得临床推广应用。

Objective To observe the clinical effect of laparoscopy combined with gonadotropin-releasing hormone agonists in the treatment of endometriosis with infertility.Methods 140 patients with endometriosis and infertility treated in our hospital from February 2018 to July 2019 were randomly divided into the control group and the observation group,with 70 cases in each group.The control group was treated with laparoscopy,and the observation group was treated with gonadotropin-releasing hormone agonist on the basis of the control group,the hormone levels,clinical effects and pregnancy status were compared between the two groups.Results Before treatment,there was no significant difference in the levels of follicle stimulating hormone(FSH),estradiol(E2)and luteinizing hormone(LH)between the two groups.After treatment,the levels of FSH,E2 and LH in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).The effective rate of the observation group was 94.29%,which was higher than 67.14%of the control group,the difference was statistically significant(P<0.05).The pregnancy rate and cycle ovulation rate in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).There was no significant difference in abortion rate between the two groups.Conclusion Laparoscopic combination of gonadotropin-releasing hormone agonists in patients with endometriosis,can effectively improve the hormone level and improve the pregnancy rate,which is worthy of clinical application.
