不同时机行玻璃体切割术对眼外伤临床疗效的影响

Clinical effect of vitrectomy in the treatment of ocular trauma

目的探讨不同时机行玻璃体切割术对眼外伤临床疗效的影响。方法选取2017年9月至2019年6月本院收治的70例(70眼)眼外伤患者作为研究对象,按照随机数字表法分为两组,每组35例。对照组于伤后1周内行玻璃体切割术治疗,研究组于伤后1~2周行玻璃体切割术治疗,比较两组临床疗效、术后12个月最佳矫正视力(BCVA)、术后1周眼压情况及术后并发症发生率。结果研究组治疗总有效率为94.29%,高于对照组的71.43%,差异有统计学意义(P<0.05)。术后12个月,研究组BCVA优于对照组,差异有统计学意义(P<0.05)。术后1周,研究组眼压≤21 mmHg占比为94.29%,高于对照组的71.43%,差异有统计学意义(P<0.05)。研究组术后并发症发生率为11.43%,低于对照组的40.00%,差异有统计学意义(P<0.05)。结论眼外伤后1~2周实施玻璃体切割术效果显著,可降低并发症发生率,治疗安全性高,值得临床推广应用。

Objective To explore the specific treatment timing of vitrectomy in patients with ocular trauma.Methods 70 patients(70 eyes)with ocular trauma treated in our hospital from September 2017 to June 2019 were selected as the research subjects,and they were divided into two groups acording to random number table method,with 35 cases in each group.The control group was treated with vitrectomy within 1 week after injury,and the study group was treated with vitrectomy within 1-2 weeks after injury,the clinical efficacy,best corrected visual acuity(BCVA)at 12 months after operation,intraocular pressure at 1 week after operation and the incidence of postoperative complications were compared between the two groups.Results The total effective rate of the study group was 94.29%,which was higher than 71.43%of the control group,and the difference was statistically significant(P<0.05).At 12 months after operation,the BCVA of the study group was better than that in the control group,and the difference was statistically significant(P<0.05).1 week after operation,the proportion of intraocular pressure≤21 mmHg in the study group was 94.29%,which was higher than 71.43%in the control group,and the difference was statistically significant(P<0.05).The incidence of postoperative complications in the study group was 11.43%,which was lower than 40.00%in the control group,the difference was statistically significant(P<0.05).Conclusion Vitrectomy performed 1-2 weeks after ocular trauma has significant effect,can reduce the incidence of complications,and has high treatment safety,which is worthy of clinical application.
