阿托伐他汀治疗急性心肌梗死的最佳剂量选择及对患者心室重构的影响

Optim um dose selection and effect of ator vast atin in patients with acute myocardial infarction

目的探讨阿托伐他汀不同给药剂量对急性心肌梗死的临床疗效及心室重构的影响。方法选取2019年1月至2021年3月本院收治的88例急性心肌梗死患者作为研究对象,按照随机抽样法分为常规组与观察组,每组44例。常规组给予20 mg阿托伐他汀治疗,观察组给予40 mg阿托伐他汀治疗,比较两组治疗前后心功能指标、心肌纤维化指标、血脂指标。结果治疗前,两组各项心功能指标、心肌纤维化指标、血脂指标比较差异无统计学意义。治疗后,观察组左室舒张末期容积、左室收缩末期容积、总胆固醇、三酰甘油、低密度脂蛋白胆固醇、基质金属蛋白酶-9均低于常规组,高密度脂蛋白胆固醇、左室射血分数、半乳凝素-3均高于常规组,差异有统计学意义(P<0.05)。结论大剂量阿托伐他汀给药对急性心肌梗死疗效更加显著,可比小剂量给药更有效地改善患者的心功能和血脂水平,抑制心室重构。

Objective To investigate effect of different doses of atorvastatin on the clinical efficacy and ventricular remodeling of patients with acute myocardial infarction.Methods 88 patients with acute myocardial infarction admitted to our hospital from January 2019 to March 2021 were selected as the research subjects.and they were randomly divided into routine group and observation group,with 44 cases in each group.The routine group was treated with 20 mg atorvastatin,and the observation group was treated with 40 mg atorvastatin,the cardiac function indexes,myocardial fibrosis indexes and blood lipid indexes were compared between the two groups before and after treatment.Results Before treatment,there was no significant difference in cardiac function indexes,myocardial fibrosis indexes and blood lipid indexes between the two groups.After treatment,the left ventricular end-diastolic volume,left ventricular end-systolic volume,total cholesterol,triglyceride,low-density lipoprotein cholesterol,and matrix metalloproteinase-9 in the observation group were lower than those in the routine group,high-density lipoprotein cholesterol,left ventricular ejection fraction,and galectin-3 were higher than those in the routine group,and the difference was statistically significant(P<0.05).Conclusions High-dose atorvastatin has a more significant curative effect on acute myocardial infarction,and can improve cardiac function and blood lipid levels and inhibit ventricular remodeling more effectively than low-dose atorvastatin.
