不同时机行微创血肿清除术治疗高血压脑出血的临床效果研究

Clinical effect of minimally invasive hematoma evacuation at different times in the treatment of hypertensive intracerebral hemorrhage

目的研究不同时机行微创血肿清除术在高血压脑出血中的应用价值。方法选取2015年1月至2019年6月该院接诊且行微创血肿清除术治疗的高血压脑出血患者90例,采用随机数表法分为A、B、C组,每组30例。A组于发病后6 h内接受手术治疗,B组为发病7~24 h内接受手术治疗,C组于发病后24 h接受手术治疗。分析3组患者治疗3个月后神经功能缺损症状的缓解情况,同时对比各组患者并发症发生率等指标。结果A组患者治疗3个月后神经功能缺损评分、日常生活能力评分均明显优于B、C组,并发症发生率均明显低于B、C组,差异均有统计学意义(P<0.05)。结论于高血压脑出血发病后的6 h内对患者施以微创血肿清除术治疗能有效改善其神经功能,促进病情康复,减少发生并发症的风险。

Objective To study the application value of minimally invasive hematoma evacuation at different times in hypertensive intracerebral hemorrhage.Methods From January 2015 to June 2019,90 patients with hypertensive cerebral hemorrhage who were treated by minimally invasive hematoma evacuation in our hospital were divided into 3 groups by random number table method,30 cases in each group.Group A received surgery within 6 hours after the onset,group B received surgery within 7-24 hours of the onset,and group C received surgery within 24 hours of the onset.The relief of symptoms of neurological deficits in the three groups was analyzed,and the complication rates and other indicators of each group were compared.Results The neurological deficit score of group A after 3 months of treatment was(6.62±1.95)points,and the score of daily living ability was(87.32±3.65)points,which were better than those of group B(13.71±3.96)points,(80.27±3.91 points)points and(21.59±4.23)points and(72.68±4.15)points in group C(P<0.05).The complication rate of group A was 6.67%,which was lower than 26.67%of group B and 40.0%of group C(P<0.05).Conclusion Within 6 hours after the onset of hypertensive intracerebral hemorrhage,minimally invasive hematoma evacuation can effectively improve the neurological function,promote the recovery of the disease,and reduce the risk of complications.
