支气管哮喘患者外周血sIL-2R、sICAM-1、CCL11水平与气道重塑的相关性分析 被引量:7

摘要 目的探究支气管哮喘患者外周血可溶性白细胞介素受体-2(sIL-2R)、可溶性细胞间黏附分子-1(sICAM-1)、CC趋化因子配体11(CCL11)水平与气道重塑的相关性。方法选取2020年5月—2021年10月我院80例支气管哮喘作为观察组,另选取同期我院体检健康者60例作为对照组。比较2组外周血sIL-2R、sICAM-1、CCL11水平及支气管管壁厚度与外径比值(T/D)、气道壁面积占气道总截面积百分比(WA%),分析外周血sIL-2R、sICAM-1、CCL11水平及气道重塑指标T/D、WA%与病情程度的关系,分析外周血sIL-2R、sICAM-1、CCL11水平与T/D、WA%的相关性,评价外周血sIL-2R、sICAM-1、CCL11水平对重度支气管哮喘的评估价值。结果观察组轻、中、重度患者外周血sIL-2R、sICAM-1、CCL11水平及T/D、WA%均高于对照组(P<0.01)。Spearman相关性分析显示,支气管哮喘患者外周血sIL-2R、sICAM-1、CCL11水平及T/D、WA%与病情程度呈正相关(P<0.01)。Pearson相关性分析显示,支气管哮喘患者外周血sIL-2R、sICAM-1、CCL11水平与气道重塑指标T/D、WA%均呈正相关(P<0.05)。受试者工作特征曲线分析显示,外周血sIL-2R、sICAM-1、CCL11联合评估重度支气管哮喘病情程度的曲线下面积为0.932,优于各指标单一评估。结论支气管哮喘患者外周血sIL-2R、sICAM-1、CCL11水平明显升高,且与气道重塑指标呈正相关,临床可通过检测其水平评估病情程度,并积极采取有效措施逆转气道重塑,改善患者预后。 Objective To explore the correlation between the levels of soluble interleukin receptor-2(sIL-2R),soluble intercellular adhesion molecule-1(sICAM-1),CC chemokine 11(CCL11)in peripheral blood and airway remodeling in patients with bronchial asthma.Methods From May 2020 to October 2021,80 patients with bronchial asthma in our hospital were selected as the observation group,and 60 healthy patients who underwent physical examination in our hospital during the same period were selected as the control group.The peripheral blood sIL-2R,sICAM-1,CCL11 levels,the ratio of bronchial tube wall thickness to outer diameter(T/D),and the percentage of airway wall area to the total airway cross-sectional area(WA%)were compared between the two groups,and the relationship between peripheral blood sIL-2R,sICAM-1,CCL11 levels,airway remodeling indicators T/D,WA%and the disease severity was analyzed.The correlation between peripheral blood sIL-2R,sICAM-1,CCL11 levels and airway remodeling indicators T/D,WA%was analyzed,and the evaluation value of peripheral blood sIL-2R,sICAM-1 and CCL11 levels in patients with severe bronchial asthma was evaluated.Results The levels of sIL-2R,sICAM-1,CCL11 in peripheral blood,T/D and WA%of mild,moderate,and severe patients in the observation group were higher than those of the control group(P<0.01).Spearman correlation analysis showed that the levels of sIL-2R,sICAM-1,CCL11 in peripheral blood,T/D,WA%in patients with bronchial asthma were positively correlated with the disease severity(P<0.01).Pearson correlation analysis indicated that the levels of sIL-2R,sICAM-1,CCL11 in peripheral blood were positively correlated with airway remodeling indicators T/D and WA%in patients with bronchial asthma(P<0.05).The receiver operating characteristic(ROC)curve showed that the area under the ROC curve of combined detection of peripheral blood sIL-2R,sICAM-1,CCL11 levels in assessing the severity of bronchial asthma was 0.932,which was larger than that of the detection of each indicator alone.Conclusion The levels of sIL-2R,sICAM-1,and CCL11 in the peripheral blood of patients with bronchial asthma are significantly increased,and they are positively correlated with airway remodeling.In clinical practice,the levels can be detected to assess the severity of the disease and effective measures can be taken to reverse airway remodeling and improve the prognosis of patients.
