双硫仑戒酒导致酒精戒断综合征延迟诊治1例

Delayed diagnose and treatment of alcohol withdrawal syndrome caused by abstaining from alcohol with disulfiram

酒精戒断综合征是酒精使用相关障碍的一种,发生于慢性酒精依赖者,由于各种原因突然减少或中断饮酒而出现的一系列精神及躯体症状。根据严重程度可表现为焦虑、出汗、震颤等轻度戒断症状,到幻觉、震颤性谵妄、酒精戒断性癫痫等严重戒断症状。提高对该综合征的认识具有重要的临床意义。本文报道1例患者自行使用双硫仑戒酒导致酒精戒断综合征,由于延迟诊治,进展为震颤性谵妄,并结合文献对该病发病机制、临床表现、疾病特征、治疗方法及可能预后等进行综述。

Alcohol withdrawal syndrome is a kind of alcohol use-related disorder.It occurs in chronic alcohol addicts and is a series of mental and physical symptoms caused by sudden reduction or interruption of drinking for various reasons.It can manifest as mild withdrawal symptoms like anxiety,sweating,and tremor,or serious withdrawal symptoms like hallucination,delirium tremens,and alcohol withdrawal epilepsy,depending on the severity.It is of great clinical significance to improve the understanding of the syndrome.This paper reports a case of alcohol withdrawal syndrome caused by the self-use of disulfiram.Due to delayed diagnosis and treatment,it developed into delirium tremens.The pathogenesis,clinical manifestations,disease characteristics,treatment methods,and possible prognosis of the disease were all reviewed in associated with the literature.
