摘要
目的:探讨青少年错[牙合]畸形与龋病发生的相关性和特异性。方法:选取2020年9月~2021年6月就读于本地区2~3所学校的998例11~14岁青少年,对其口腔情况进行检查并记录错[牙合]畸形、龋病情况,分析青少年错[牙合]畸形与龋病发生的相关性和特异性。结果:998例青少年239例为龋病无错[牙合]畸形,184例为无龋病有错[牙合]畸形,195例为有龋病并错[牙合]畸形,380例为无龋无错[牙合]畸形;错[牙合]畸形AngleⅠ类占42.74%,Ⅱ类占23.75%,Ⅲ类占33.51%。单因素分析显示每天刷牙次数、刷牙时间、刷牙方式、吃甜食行为、错[牙合]畸形、牙龈出血、最近一次看牙时间差异有统计学意义(P<0.05);多因素Logistic回归分析显示每天刷牙次数、正确刷牙、最近一次看牙时间、吃甜食行为、错[牙合]畸形是青少年龋病发生的重要影响因素(P<0.05)。有龋病并错[牙合]畸形组龋失补指数(DMFT),牙齿美学指数(DAI)均高于其余3组(P<0.05);错[牙合]畸形与DMFT、DAI呈正相关(P<0.05)。受试者工作特征(ROC)曲线分析显示DMFT和DAI曲线下面积分别为0.762、0.729。结论:青少年错[牙合]畸形与龋病发生密切相关,其错[牙合]畸形青少年龋失补指数和牙齿美学指数较高,该两项指数可作为预测龋病发生的因素,可为临床诊断龋病提供参考。
Objective:To explore the correlation and specificity between malocclusion and caries in adolescents.Methods:A total of 998 adolescents aged 11~14 years in 2~3 schools of the area were enrolled from September 2020 to June 2021.Their oral conditions were examined to record malocclusion an caries.The correlation and specificity between malocclusion and caries in adolescents were analyzed.Results:Of the 998 adolescents, there were 239 cases with caries, 184 cases with malocclusion, 195 cases with caries and malocclusion, and 380 cases without caries or malocclusion.The proportions of cases with Angle Class Ⅰ,Ⅱ and Ⅲ malocclusion were 42.74%,23.75% and 33.51%,respectively.Univariate analysis showed that the differences in caries among adolescents with different times of daily tooth brushing, brushing time, brushing modes, eating sweets behavior, malformation, gingival bleeding and the last time of dental visit were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that times of daily tooth brushing, correct brushing, the last time of dental visit, eating sweets behavior and malformation were influencing factors of caries in adolescents(P<0.05).The decayed/missing/filled teeth(DMFT)and dental aesthetic index(DAI)in caries-malformation group were higher than those in the other three groups(P<0.05).Malocclusion was positively correlated with DMFT and DAI(P<0.05).The receiver operating characteristic(ROC)curve analysis found that the areas under the curve values of DMFT and DAI were 0.762 and 0.729.Conclusion:Adolescent malocclusion is closely related to caries.DMFT and DAI are relatively higher in malocclusion adolescents.DMFT and DAI can be applied to predict caries, which can provides reference in the clinical diagnosis of caries.
