糖尿病对农村老年人轻度认知障碍的影响

Impacts of diabetes on mild cognitive impairment in rural older residents

目的了解农村老年人轻度认知障碍现状,并分析其与糖尿病之间的关联。方法对1897名农村老年人进行了现场问卷调查,其中有效问卷为1857份。调查内容包括基本人口学信息、糖尿病等慢性疾病信息以及蒙特利尔认知评估量表。结果在1857名老年人中,被筛查为轻度认知障碍的有350(18.85%)人;女性、高龄、受教育程度低、未婚、独居、在家休息及患有糖尿病的老年人有更高的轻度认知障碍患病率。逐步logistic回归分析结果显示,在控制了年龄(χ^(2)=97.98,P<0.001)、教育程度(χ^(2)=136.92,P<0.001)和脑卒中(χ^(2)=3.77,P=0.052)等因素后,糖尿病对轻度认知障碍的影响并不显著(χ^(2)=3.62,P=0.057)。结论轻度认知障碍在农村老年人群中的患病率较高,其中年龄和教育程度是轻度认知障碍主要的影响因素,糖尿病等慢性疾病对轻度认知障碍的影响并不显著。

Objective To investigate the prevalence of mild cognitive impairment in rural older residents,and to explore the association between mild cognitive impairment and diabetes.Methods A total of 1897 older residents were investigated with on-site questionnaires,1857 copies of which were valid.The survey contents included basic demographic characteristics,information about chronic diseases such as diabetes as well as the Montreal Cognitive Assessment.Results Among the 1857 investigated subjects,350(18.85%)individuals were screened and confirmed with mild cognitive impairment.Those who were female,with older age,low education level,unmarried,living alone,not employed and suffering from diabetes had higher rates of developing mild cognitive impairment.The results of stepwise logistic regression indicated that diabetes did not had a significant impact(χ~2=3.62,P=0.057)on mild cognitive impairment after controlling the factors of age(χ~2=97.98,P<0.001),education level(χ~2=136.92,P<0.001)and stroke(χ~2=3.77,P=0.052).Conclusions The prevalence of mild cognitive impairment in rural older residents is high,and age and education level are the main influencing factors for mild cognitive impairment,while the impact of chronic diseases including diabetes on mild cognitive impairment is not significant.
