肩三针温针灸联合蠲痹汤治疗风寒湿痹型肩周炎临床研究

Clinical study on shoulder three-needle warm acupuncture combined with Juanbi Decoction in treatment of wind-cold-damp-arthritis type of periarthritis of shoulder

目的观察肩三针温针灸联合蠲痹汤联合治疗风寒湿痹型肩周炎(PS)临床疗效。方法将我院收治的PS患者150例随机分为对照组与观察组各75例。对照组予以口服布洛芬治疗,观察组予以肩三针温针灸联合蠲痹汤治疗。治疗后统计两组患者的临床总有效率,于治疗前后测疼痛视觉模拟评分法(VAS)评分、肩关节功能Melle评分及日常生活能力(ADL)评分,同时检测C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)水平。结果治疗后,观察组患者总有效率高于对照组(P<0.05);两组患者VAS、Melle降低,观察组患者VAS、Melle、ADL低于对照组(P<0.05);两组患者IL-6、TNF-α、CRP降低,观察组患者IL-6、TNF-α、CRP低于对照组(P<0.05)。结论肩三针温针灸、蠲痹汤联合治疗风寒湿痹型PS患者,疗效显著,可降低疼痛,扩大肩关节活动度,提高日常生活能力,机制可能是降低体内炎症因子水平。

Objective To observe the clinical effect of Shoulder three-needle warm acupuncture combined with Juanbi Decoction in treatment of wind-cold-damp-arthritis type of periarthritis of shoulder(PS).Methods A total of 150 patients with PS were randomly divided into control group and observation group.Control group was treated with oral ibuprofen,and observation group was treated with Shoulder three-needle warm acupuncture combined with Juanbi Decoction.After treatment,the total cl inical effective rate of the two groups was counted,and the Visual analogue scale(VAS)score,Shoulder joint function Melle and Activity of daily living(ADL)score,C-reactive protein(CPR),tumor necrosis factor-α(TNF-α),interleukin(IL)-6 levels were measured before and after treatment.Results After treatment,the total effective rate of observation group was higher than of control group(P<0.05);the VAS and Melle scores of both groups decreased,and the VAS,Melle and ADL scores of observation group were lower than those of control group(P<0.05);the levels of IL-6,TNF-αand CRP in both groups decreased,which were lower in observation group than those in control group(P<0.05).Conclusion Juanbi Decoction combined with shoulder three-needle warm acupuncture has a significant effect in treatment of PS due to wind-cold-dampness-arthritis.It can reduce pain,expand the range of motion of the shoulder joint,and improve the ability of daily living,the mechanism many be related to reduce the level of inflammatory factors.
