摘要
目的:系统评价浓缩生长因子应用于牙齿拔除后牙槽窝位点保存的效果,为临床中应用浓缩生长因子减少拔牙位点牙槽骨吸收,促进牙槽骨再生提供理论支持。方法:在PubMed、EMbase、CBM、Web of Science、Cochrane Library、维普、万方和中国知网等中英文数据库检索相关文献,收集所有将CGF用于拔牙后牙槽窝位点保存的随机对照试验。每位研究者分别提取相关数据,使用Cochrane偏倚风险评估工具评估所纳入文献的质量及偏倚风险。应用RevMan 5.3软件对测量数据行meta分析。结果:共检索出70篇文献,最终纳入7篇符合要求的随机对照试验,共276名患者,292个拔牙位点;拔牙窝用CGF复合人工骨材料处理与拔牙窝仅使用人工骨材料或自然愈合处理在拔牙后颊侧牙槽骨吸收高度具有显著差异(3个月:SMD=-0.86;6个月:SMD=-2.52;P<0.05);腭侧牙槽骨吸收高度具有显著差异(3个月:SMD=-0.85;6个月:SMD=-1.27;P<0.05);拔牙后使用CGF组与未使用CGF组相比,牙槽骨的吸收宽度存在显著差异(SMD=-1.43,P<0.05)。结论:CGF复合人工骨材料进行拔牙后位点保存可显著降低牙槽骨吸收,提高牙槽骨成骨的疗效。
Objective:To systematically evaluate the effect of concentrated growth factor on the preservation of alveolar and fossa sites after tooth extraction,and to provide theoretical support for clinical application of concentrated growth factor to reduce the absorption of alveolar bone at tooth extraction sites and promote alveolar bone regeneration.Methods:Relevant literatures were searched in PubMed,EMbase,CBM,Web of Science,Cochrane Library,VIP,WANFANG,CNKI databases,and all randomized controlled trials using CGF in the preservation of alveolus and fossa sites after tooth extraction were collected.Relevant data were extracted for each investigator and the quality and risk of bias were assessed using the Cochrane bias risk assessment tool.Meta-analysis of the measured data was performed using RevMan 5.3 software.Results:A total of 70 literatures were retrieved,and 7 qualified randomized controlled trials were included,involving 276 patients and 292 extraction sites.The alveolar bone resorption height of the buccal alveolar bone was different between the treatment with CGF combined with artificial bone material and the treatment with natural healing after tooth extraction(3 months:SMD=-0.86,6 months:SMD=-2.52,P<0.05)had significant significance.Palatal alveolar bone resorption height was significantly different(3 months:SMD=-0.85,6 months:SMD=-1.27,P<0.05)had significant significance.The absorption width of alveolar bone was significantly different(SMD=-1.43,P<0.05)has significant significance.Conclusion:The use of CGF composite artificial bone material for site preservation after tooth extraction can significantly reduce the absorption of alveolar bone,and improve the final effect of alveolar bone osteogenesis.
