针刀配合松解手法治疗肩关节周围炎的临床效果

Clinical effect of acupotomy combined with loosening manipulation in the treatment of scapulohumeral periarthritis

目的探讨针刀配合松解手法治疗肩关节周围炎(SP)的临床效果。方法将2018年1月至2021年2月我科收治的160例SP患者随机分为对照组(80例,松解手法+西药外敷+功能锻炼)和观察组(80例,在对照组基础上配合针刀)。比较两组的临床效果。结果观察组治疗总有效率高于对照组(P<0.05)。治疗后,两组的Constant-Murley肩关节功能评分(CMS)高于治疗前,疼痛数字评分(NRS)低于治疗前,且观察组优于对照组(P<0.05)。治疗后,两组的肿瘤坏死因子-α(TNF-α)、前列腺素E_(2)(PGE_(2))、一氧化氮(NO)、一氧化氮合酶(NOS)水平低于治疗前,且观察组低于对照组(P<0.05)。结论针刀配合松解手法治疗SP能有效缓解肩周疼痛、肿胀并改善肩关节功能,值得临床推广应用。

Objective To explore the clinical effect of acupotomy combined with loosening manipulation in the treatment of scapulohumeral periarthritis(SP).Methods A total of 160 patients with SP treated in our department from January 2018 to February 2021 were randomly divided into control group(80 cases,loosening manipulation+external application of western medicine+functional exercise)and observation group(80 cases,acupotomy on the basis of the control group).The clinical effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the Constant-Murley shoulder function score(CMS)in the two groups was higher than that before treatment,and the numerical rating scales(NRS)was lower than that before treatment,and those in the observation group were better than the control group(P<0.05).After treatment,the levels of tumornecrosis factor-α(TNF-α),prostaglandin E_(2)(PGE_(2)),nitric oxide(NO),nitric oxide synthase(NOS)in the two groups were lower than those before treatment,and those in the observation group were lower than the control group(P<0.05).Conclusion Acupotomy combined with loosening manipulation in the treatment of SP can effectively relieve pain and swelling around the shoulder and improve the function of the shoulder joint,which is worthy of clinical promotion and application.
