福建省厦门市海沧区居民大肠癌筛查的优化效果分析

Optimization of Colorectal Cancer Screening in Haicang District,Xiamen City,Fujian Province

目的探讨福建省厦门市海沧区居民大肠癌筛查的优化效果。方法全市统一安排,海沧区2019年、2020年和2021年分别启动大肠癌筛查工作,2021年通过深入社区进行培训宣传,改进手机app录入信息的不足,由网格员在发放试管的同时指导填写数据,运用体验管理理论:改进医院品牌(加挂厦门医学院附属医院)影响力、社区组织有序、网格员或村医现场讲解、筛查流程改进等措施,推进大肠癌筛查工作的全面实施,并对2021年度和2020年度、2019年度该辖区大肠癌筛查情况进行比较。结果2021年初筛阳性率、肠镜检查率均明显高于2020年、2019年(均P<0.01)。结论大肠癌筛查对于大肠癌早期发现以及早期治疗有积极作用,但是初筛阳性居民肠镜检查顺应性较差,采取恰当的优化措施可显著提升大肠癌社区筛查中初筛阳性者的肠镜检查率,有助于大肠癌及癌前病变的早发现、早治疗。

Objective To explore the optimized effect of colorectal cancer screening in Haicang District residents of Fujian Province.Methods Under the unified arrangement of the city,Haicang District will start colorectal cancer screening work in 2019,2020 and 2021respectively.In 2021,through in-depth community training and publicity,the insufficiency of mobile app information input will be improved.Grid staff will guide data filling while handing out test tubes,and the theory of experience management will be applied:to promote the comprehensive implementation of colorectal cancer screening,measures such as improving the influence of the hospital brand(adding the affiliated hospital of Xiamen Medical University),orderly community organization,on-site explanation by grid staff or village doctors,and improvement of screening process were taken to promote the comprehensive implementation of colorectal cancer screening,and the colorectal cancer screening situation in the area in 2021,2020 and 2019 was compared.Results the positive rates of 2021 screening and colonoscopy were significantly higher than those in 2020(P<0.01).Conclusion Colorectal cancer screening has a positive effect on early detection and treatment of colorectal cancer,but the compliance with colonoscopy examination is poor in residents with positive initial screening,appropriate optimization measures can significantly increase the rate of colonoscopy in community screening for colorectal cancer,and contribute to the early detection and treatment of colorectal cancer and precancerous lesions.
