石膏托结合小夹板在桡骨远端骨折保守治疗中的疗效观察

Curative Effect Observation of Plaster Support Combined With Small Splint in Conservative Treatment of Distal Radius Fracture

目的研究保守治疗桡骨远端骨折中单纯夹板固定与石膏托结合小夹板固定在临床应用的疗效。方法选择我院2020年1月—2021年12月收治的60例桡骨远端骨折患者,实施奇偶数分组模式将患者平均分为试验组和对照组,每组30例,试验组患者采用手法复位石膏托结合小夹板固定治疗,对照组予以手法复位结合小夹板固定治疗。治疗1个月后通过比较两组患者的治疗效果、骨折再移位率、腕关节功能评分、疼痛评分等评估患者的预后。结果术后60例患者均完成随访,平均随访时间(8.4±2.6)周。试验组中治疗总有效率(96.67%vs.76.67%),骨折再移位率(3.33%vs.20.00%),腕关节功能评分(4.28±2.67)分vs.(8.58±4.37)分,生活质量评分(83.26±5.32)分vs.(72.48±8.47)分,VAS疼痛评分(1.52±1.37)分vs.(3.28±1.67)分等均优于对照组,且差异有统计学意义(P<0.05)。结论在保守治疗桡骨远端骨折患者中采用石膏托结合小夹板固定或单纯夹板固定,二者都可以取得一定的疗效,但石膏托结合小夹板固定的临床治疗效果更好,其在改善患者腕关节功能和疼痛、预防骨折再移位等方面更有效。

Objective To study the clinical effect of simple splint fixation and plaster support combined with small splint fixation in conservative treatment of distal radius fractures.Methods A total of 60 patients with distal radius fractures admitted to our hospital from January 2020 to December 2021 were selected.The odd and even number grouping mode was implemented to divide the patients into the test group and the control group,30 cases in each group.The test group patients were reduced by manual reduction.Plaster support combined with small splint fixation treatment,the control group received manual reduction splint fixation treatment.One month after treatment,the prognosis of the patients was evaluated by comparing the treatment effect,fracture re-displacement rate,wrist function score,pain score,etc.between the two groups.Results All 60 patients completed follow-up,with an average follow-up time of(8.4±2.6)weeks.The total effective rate of treatment in the trial group(96.67%vs.76.67%),fracture relocation rate(3.33%vs.20.00%),wrist function score(4.28±2.67)points vs.(8.58±4.37)points,quality of life score(83.26±5.32)points vs.(72.48±8.47)points,VAS pain score(1.52±1.37)points vs.(3.28±1.67)points were better than the control group,and the differences were statistically significant(P<0.05).Conclusion In the conservative treatment of patients with distal radius fractures,plaster support combined with small splint fixation or simple splint fixation can achieve a certain effect,but the clinical treatment effect of plaster support combined with small splint fixation is better,and it can improve the patient’s wrist.It is more effective in terms of joint function and pain,prevention of fracture and re-displacement.
