高强度聚焦超声治疗子宫肌瘤的临床应用研究

Clinical Application of High-intensity Focus Ultrasound to Treat Uterine Fibroids

目的探讨对子宫肌瘤应用高强度聚焦超声(HIFU)治疗的价值。方法纳入2020年1月—2021年1月收治的80例子宫肌瘤患者按照随机数字表法分成两组,40例对照组应用传统子宫肌瘤剔除术治疗,40例观察组患者应用HIFU治疗,对比两组围术期指标、治疗前与治疗1周后促卵泡生成素(FSH)、雌二醇(E2)、黄体生成素(LH)水平变化、治疗前与治疗6个月后生活质量(QOL评分)与子宫肌瘤症状(UFS评分)变化及并发症发生情况。结果两组手术时间差异无统计学意义(P>0.05),观察组术中出血量少于对照组,术后下床活动时间与住院时间短于对照组(P<0.05);治疗前后两组血清FSH、E2、LH水平比较差异无统计学意义(P>0.05);治疗后患者QOL评分较治疗前提高,UFS评分较治疗前降低,观察组评分变化较对照组明显(P<0.05);观察组2.50%的并发症总发生率明明显低于对照组17.50%的总发生率(P<0.05)。结论对子宫肌瘤应用HIFU治疗的创伤小且术后恢复快,且对体内性激素水平的影响小,同时可改善患者子宫肌瘤症状及提高生活质量,并发症发生率低。

Objective To explore the value of uterine fibroids to apply high-strength focus ultrasound(HIFU)treatment.Methods 80 cases of uterine fibroids injured in January 2002 to January 2021 were divided into two groups in accordance with the random number,and 40 cases of control group were used to apply traditional uterine fibroids.40patients with observation group used HIFU treatment.Comparison of the two groups of perioperative indicators,before treatment with treatment 1 week,ovulatory is biochemical(FSH),estradiol(E2),yellow body elementary elevatin(LH)level change,before treatment and treatment of 6 months later Changes of uterine fibroids(UFS ratings)and complications.Results There was no significant difference in operation time(P>0.05),and the blood loss during the observation group was less than the control group.After the operation,the time and hospitalization time was shorter than the control group(P<0.05);two groups of serum before and after treatment FSH,E2,LH levels have no statistical significance(P>0.05);QOL scores after treatment have improved before treatment,and UFS scores have been reduced before treatment,and the observation group score is significant than the control group(P<0.05);observation the total incidence of complications in groups was significantly lower than the total incidence of 17.50%in the control group(P<0.05).Conclusion Treatment of HIFU treatment for uterine fibroids is small and fast postoperative recovery,and small in vivo hormone levels,and improve the symptoms of uterine fibroids in patients and improve their quality of life,and the incidence of complications is low.
