宁心通痹汤联合阿利沙坦酯治疗冠心病不稳定型心绞痛患者的临床效果

Clinical effect of Ningxin Tongbi Decoction combined with Allisartan Isoproxil on patients with unstable angina pectoris of coronary heart disease

目的探讨宁心通痹汤联合阿利沙坦酯治疗冠心病不稳定型心绞痛患者的临床效果。方法选取2019年7月至2021年9月厦门市中医院心内科收治的150例冠心病不稳定型心绞痛患者作为研究对象。采用随机数字表法分为对照组(n=75)和观察组(n=75)。两组采用阿利沙坦酯治疗,观察组加用宁心通痹汤。比较两组治疗前后中医证候总积分、心肌缺血状态、心率变异性、治疗后心绞痛发生情况及治疗期间不良反应发生情况。结果观察组治疗后中医证候总积分低于对照组,差异有统计学意义(P<0.05);观察组治疗后的窦性心搏RR间期标准差、RR间期平均值标准差和相邻RR间期差值均方根水平均高于对照组,差异有统计学意义(P<0.05);观察组治疗后的心肌缺血总负荷、QT离散度、JT离散度均低于对照组,差异有统计学意义(P<0.05);观察组治疗后心绞痛发作频率、硝酸甘油用量低于对照组,持续时间短于对照组,差异有统计学意义(P<0.05);两组各项不良反应发生率比较,差异无统计学意义(P>0.05)。结论宁心通痹汤联合阿利沙坦酯能有效改善冠心病不稳定型心绞痛患者临床症状,提高心率变异性,具有较高的临床疗效。

Objective To explore the clinical effect of Ningxin Tongbi Decoction combined with Allisartan Isoproxil in the treatment of unstable angina pectoris of coronary heart disease.Methods A total of 150 patients with coronary heart disease unstable angina pectoris admitted to the Department of Cardiology,Xiamen Hospital of Traditional Chinese Medicine from July 2019 to September 2021 were selected as the research subjects.The group was divided into control group(n=75)and observation group(n=75)by random number table method.The two groups were treated with Allisartan Isoproxil,and the observation group was treated with Ningxin Tongbi Decoction.The total score of traditional Chinese medicine syndromes,myocardial ischemia,heart rate variability,angina pectoris after treatment and adverse reactions during treatment were compared between the two groups before and after treatment.Results The total score of traditional Chinese medicine syndrome in the observation group was lower than that in the control group after treatment,and the difference was statistically significant(P<0.05).The standard deviation of RR interval,mean standard deviation of RR interval and root mean square root difference of adjacent RR interval in the observation group were higher than those in the control group after treatment,the differences were statistically significant(P<0.05).The total myocardial ischemia load,QT dispersion and JT dispersion in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The frequency of angina pectoris and nitroglycerin dosage in the observation group were lower than those in the control group,and the duration was shorter than that in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Ningxin Tongbi Decoction combined with Alisartan Isoproxil can effectively improve the clinical symptoms and heart rate variability of patients with unstable angina pectoris of coronary heart disease.
