叶天士辨治耳病思想探析

YE Tian-shi’s thought in treating ear diseases

文章通过对《临证指南医案》所载耳病医案的分析,总结出叶天士辨治耳病以虚、实为两大纲。虚证病位主要在肾,有下元亏虚;实证病位主要在胆,有风温上郁、暑热闭窍、风火侵窍、气闭耳窍;虚实相兼则有素体阴亏,外受风温;心肾两亏,胆火上炎;肾阴久亏,肝风上旋,并分析了每一证候用药特点,以期能为指导医者临床治疗耳病提供一定的参考。

Based on the analysis of the medical cases of ear diseases in Linzheng Zhinan Yi’an,it is concluded that the two outlines of YE Tian-shi’s diagnosis and treatment of ear disease are deficiency and excess.The syndrome of deficiency mainly occurs in the kidney,with the lack of xiayuan.The syndrome of excess mainly occurs in the gallbladder,with the invasion of wind and warmth,the heat shuts off the ear,the invasion of wind and fire,qi blocking the ear.The syndrome of deficiency and excess,with normal yin deficiency of the body and the invasion of wind and warmth outside,the deficiency of heart and kidney and the fire up of the gallbladder.Yin deficiency of the kidney and the invasion of wind of the liver.The characteristics of the drugs of each syndrome were analyzed in order to provide some references for guiding doctors in clinical treatment of ear diseases.
