摘要
目的:通过观察脊柱推拿对慢性下腰痛(CLBP)模型大鼠后肢步态参数的影响,探讨脊柱推拿对CLBP模型大鼠运动功能障碍的改善情况。方法:将32只雄性SD大鼠随机分为空白组、假手术组、CLBP模型组和推拿治疗组,每组8只。通过外部链接固定钢板固定大鼠腰椎制备CLBP模型,固定2周后成模。推拿治疗组大鼠在造模后第15天开始给予按揉两侧L3~S1夹脊穴进行推拿治疗,共10穴,每穴治疗1 min。设定推拿按揉力度5 N,频率2 Hz,10 min/次,1次/d,连续干预2周。运用CatWalk步态分析系统检测4组大鼠造模前及干预后第1、3、7、10、14天步态参数的改变。结果:造模前,4组大鼠的步态参数比较差异均无统计学意义。干预后第1天开始,CLBP模型组和推拿治疗组大鼠的站立时相持续时间、举步时相持续时间和步行持续时间较空白组均显著上升(P<0.01),干预后第7或10或14天,推拿治疗组大鼠这3项步态参数出现显著下降(P<0.05,P<0.01);干预后第1天开始,CLBP模型组和推拿治疗组大鼠的举步速度较空白组显著下降(P<0.01),干预后第10、14天,推拿治疗组大鼠举步速度较CLBP模型组显著上升(P<0.05,P<0.01)。结论:脊柱推拿可以影响CLBP模型大鼠后肢步态参数,从而改善其运动功能。
Objective:To observe the effect of spinal Tuina on the hindlimb gait parameters of chronic low back pain(CLBP)model rats,and to discuss the improvement of spinal Tuina on the motor dysfunction of CLBP model rats.Methods:Thirty-two male SD rats were randomly divided into naive group,sham operation group,CLBP model group and Tuina treatment group,with 8 rats in each group.The CLBP model was prepared by externally fixing the steel plate to fix the rat lumbar spine,and the model was formed after two weeks of fixation.On the 15th day after modeling,the rats in the Tuina treatment group were given L3 to S1 Jiaji acupoints on both sides for treatment,total of 10 points and each point was treated for 1 min.The force of manipulation was set to 5 N,frequency 2 Hz,10 min/time,1 time/day,continuous intervention for two weeks.The CatWalk animal gait analysis system was used to detect the changes of gait parameters of four groups of rats before modeling and after intervention 1,3,7,10,14 days.Results:Before modeling,the gait parameters of the four groups of rats were not statistically significant.After intervention 1 day,the standing phase duration,swing phase duration and walking duration of the CLBP model group and the Tuina treatment group were significantly higher than those of the naive group(P<0.01).On the 7th,10th,or 14th days after the intervention,the three gait parameters of the rats in the Tuina treatment group decreased significantly than the CLBP model group(P<0.05,P<0.01).After modeling,the rats in the model group and the Tuina treatment group had a significantly lower pace than the naive group(P<0.01).On the 10th and 14th days after the intervention,the pace of the rats in the Tuina treatment group was significantly higher than that in the naive group(P<0.05,P<0.01).Conclusion:Spinal Tuina can influence the hindlimb gait parameters of CLBP model rats,thereby improving their motor function.
