中医综合疗法治疗糖尿病足溃疡的临床疗效观察

Clinical observation on diabetic foot ulcer treated by TCM comprehensive therapy

目的:观察周氏骨髓炎疗法中医药方剂克骨汤辨证内服结合中医外治法治疗糖尿病足溃疡的诊疗方案的临床疗效和安全性,为进一步优化诊疗方案奠定基础。方法:选取2018年3月—2021年6月于杭州富阳中医骨髓炎医院收治的糖尿病足溃疡患者114例,随机分为对照组和观察组,每组57例。对照组采取西医常规治疗,观察组在对照组治疗基础上加用克骨汤辨证内服,复方浸透紫草油纱条外敷以及红光灸法等综合治疗措施。两组疗程均为3个月。比较两组患者的治疗有效率、踝肱比指数(ABI)、足部临床症状变化。结果:观察组治疗总有效率为92.98%(53/57)显著高于对照组的77.19%(44/57)(P<0.05)。治疗后,观察组ABI值、足部临床症状阴性率显著高于对照组(P<0.01,P<0.05)。结论:克骨汤联合复方浸透紫草油纱条外敷、红光灸法的综合疗法治疗糖尿病足溃疡效果显著。

Objective:To observe the clinical efficacy and safety of the traditional Chinese medicine prescription Kegu Decoction of ZHOUs’osteomyelitis therapy based on syndrome differentiation combined with external treatment of traditional Chinese medicine for the treatment of diabetic foot ulcer,and to lay a foundation for further optimization of the diagnosis and treatment plan.Methods:A total of 114 patients with diabetic foot ulcers who were admitted to Hangzhou Fuyang Traditional Chinese Medicine Osteomyelitis Hospital from March 2018 to June 2021 were selected and randomly divided into control group and observation group with 57 cases in each group.The control group received conventional western medicine treatment,while the observation group received comprehensive measures such as oral administration of Kegu Decoction based on syndrome diflerentiation,external application of compound soaked arnebia oil gauze,and red light moxibustion on the basis of the treatment for control group.The course of treatment in both groups was 3 months.The treatment efficiency,ankle-brachial index(ABI),and foot symptoms were compared between the two groups.Results:The effective rate of treatment in the observation group was 92.98%(53/57),which was significantly higher than that in the control group,which was 77.19%(44/57)(P<0.05).After treatment,the ABI value and the improvement rate of foot symptoms in the observation group were significantly higher than those in the control group(P<0.01,P<0.05).Conclusion:The combined therapy of Kegu Decoction combined with compound soaked arnebia oil gauze for external application and red light moxibustion has a more significant effect on diabetic foot ulcers.
