多层螺旋CT检测指标评估进展期结直肠癌周围组织受侵情况的价值

The Value of Multi-slice Spiral CT Norms in Evaluating the Invaded Status of the Tissue Nearby Advanced Colorectal Cancer

目的:探讨多层螺旋CT(MSCT)检测指标用于评估进展期结直肠癌周围组织受侵情况的价值。方法:以2019年7月至2020年6月于我院行手术治疗、术后病理学证实为进展期结直肠癌且术前接受MSCT检查的52例患者为研究对象。其中术中证实肿瘤病灶周围组织受侵24例(A组),未受侵28例(B组)。比较2组MSCT检测指标(肿瘤病灶与周围组织或脏器接触面的形态分型、长度、清晰度,以及肿瘤病灶厚度)。结果:2组患者肿瘤病灶与周围组织或脏器的接触面的长度及肿瘤病灶厚度比较无明显差异,P>0.05。2组患者肿瘤病灶与周围组织或脏器的接触面的形态分型及清晰度方面比较差异有统计学意义,P<0.05。接触面清晰度判断周围组织受侵情况的敏感性为87.50%,特异性为82.14%。Ⅳ型接触面判断周围组织受侵情况的敏感性为58.33%,特异性为100%。结合窗宽调节、窗位调节及多平面重组技术可准确判断结直肠癌病灶对周围组织结构的侵犯情况。结论:MSCT检查所示肿瘤病灶与周围组织或脏器接触面的形态分型及清晰度,结合窗宽调节、窗位调节及多平面重组技术,对评估进展期结直肠癌周围组织受侵情况具有一定价值。

Objective To explore the value of multi-slice spiral CT(MSCT)in evaluating the invaded status of surrounding tissue to advanved colorectal cancer.Methods In the study enrolled the 52patients who received surgery(2019-07-2020-06)in author’s hospital,then,were confirmed as advanced colorectal cancer by postoperatively pathological exam.,and,at preoperation they were all inspected by MSCT,including group A(24cases,their surrounding tissue to neoplasm-lesion invaded)and group B(28cases,without invaded tissue);Then,compared both groups’MSCT norms:the form-type,length and defining power in the touch-surface between lesion and surrounding tissue or organs,as well as neoplasm’s thickness.Results In the length and thickness there were no significant difference between the two groups(P>0.05);but in form-types,and defining power there were significant difference between the two groups(P<0.05).The sensitivity and specificity of the defining power for evaluating invaded status of surrounding tissue were respectively 87.5%and 82.14%,that ofⅣgrade touch-surface,meanwhile,58.33%and 100%,if combined with window-width regulation,whindow-site regulation,and multi-planar reconstruction skill,it could accuracyly judge invaded status of surrounding tissue.Conclusion The form-types and defining power of touch-surface shown by MSCT,combined with window-width and window-site regulation,as well as multi-planar reconstruction skill have certain value for evaluating the invaded status of surrounding tissue to advanced colorectal cancer.
