高分辨MRI评估直肠癌术前T、N分期的价值分析

Value Analysis of High-resolution MRI in Preoperative T and N Staging of Rectal Cancer

目的:探讨高分辨MRI评估直肠癌术前T、N分期的价值。方法:回顾2019年1月至2021年6月于我院接受手术治疗且术前通过高分辨MRI检查评估T、N分期的57例直肠癌患者资料,以术后病理结果为标准,分析高分辨MRI评估直肠癌T、N分期的准确性。结果:高分辨MRI评估T分期结果与病理结果一致49例,不一致8例,准确率为85.96%,Kappa值为0.745,表明高分辨MRI评估T分期结果与病理结果一致性好。高分辨MRI评估N分期结果与病理结果一致40例,不一致17例,准确率为70.18%,Kappa值为0.558,表明高分辨MRI评估N分期结果与病理结果一致性较好。结论:高分辨MRI可较好地显示直肠癌浸润深度、淋巴结转移情况,用于评估直肠癌术前T、N分期具有一定价值。

Objective To evaluate the value of high-resolution MRI in preoperative T and N staging of rectal cancer.Methods The data of 57patients with rectal cancer who underwent surgery in author’s hospital from January 2019to October 2021and were evaluated for T and N staging by high-resolution MRI before operation were retrospectively analyzed.The accuracy of high-resolution MRI in the evaluation of T and N staging of rectal cancer was analyzed based on the postoperative pathological results.Results The results of high-resolution MRI in T staging were consistent with pathological results in 49cases and inconsistent in 8cases.The accuracy rate was 85.96%,and the Kappavalue was 0.745,indicating that the results of high-resolution MRI in T staging were better consistent with pathological results.The results of high-resolution MRI in N staging were consistent with pathological results in 40cases and inconsistent in17cases.The accuracy rate was 70.18%,and the Kappavalue was 0.558,indicating that the results of high-resolution MRI in N staging were good consistent with pathological results.Conclusion High-resolution MRI can better display the depth of invasion and lymph node metastasis of rectal cancer,and has certain value in preoperative T and N staging of rectal cancer.
