智慧医疗背景下五元联动整合照护模式的构建在老年慢性病患者中的应用研究

Research on the application of five-element linkage and integrated care model in elderly patients with chronic diseases under the background of smart medical treatment

目的构建智慧医疗背景下老年慢性疾病的五元联动整合照护模式,评价该模式的应用效果。方法选择老年慢性病患者690例,按照智慧医疗背景下老年慢性病五元联动整合照护模式建立前后,将患者分为对照组326例与观察组364例。对照组给予常规护理,观察组实施老年慢性病五元联动整合照护,比较两组护理效果。结果观察组老年慢性病患者再入院率降低(χ^(2)=14.663,P<0.001),患者满意度提高(t=-3.387,P<0.001),生活质量提高(t=-17.594,P<0.001),人均年医疗费用支出明显降低(t=14.559,P<0.001)。结论基于智慧医疗背景构建老年慢性疾病五元联动整合照护模式能降低老年慢性病患者再入院率,提高了患者满意度、生活质量和减少了医疗费用支出。

Objective To explore the construction of five-element linkage and integrated care model for elderly chronic diseases under the background of smart medical treatment and evaluate the application effect of the model.Methods A total of 690 senile patients with chronic diseases were selected and divided into control group(n=326)and observation group(n=364)before and after the establishment of five-element linkage and integrated care model for senile chronic diseases under the background of intelligent medical treatment.The control group was given routine care,and the observation group was given five-element linkage and integrated care model,and the nursing effects of the two groups were compared.Results In the observation group,the readmission rate of elderly patients with chronic diseases was decreased(χ^(2)=14.663,P<0.001),patient satisfaction was improved(t=-3.387,P<0.001),and quality of life was improved(t=-17.594,P<0.001).The per capita annual medical expenditure was significantly lower(t=14.559,P<0.001).Conclusion Based on the background of smart medical care,the five-element linkage and integrated care model for elderly patients with chronic diseases can reduce the readmission rate of elderly patients with chronic diseases,improve patient satisfaction,quality of life and reduce medical expenses.
