摘要
目的探讨LP-PLA2在初诊断2型糖尿病微血管病变患者中的水平变化及相关性。方法选取泰州市人民医院内分泌科2020年1月至2021年1月住院或门诊初次诊断的糖尿病患者156例(糖尿病组),按照有无糖尿病性微血管病变分为非糖尿病性微血管病变组(A组),糖尿病性微血管病变组(B组),其中糖尿病性微血管病变组包括糖尿病性肾病和糖尿病性视网膜病变;选取14例健康体检者做对照为正常组(C组);分别检测脂蛋白相关磷脂酶A2 LP-PLA2指标外,同时测定体重指数,腰围、肝肾功能、血糖、血脂、空腹C肽、糖化血红蛋白、血清胱抑素、β2-微球蛋白、尿ACR等,分析LP-PLA2在初诊断2型糖尿病性微血管病变患者中的水平变化及相关性。结果糖尿病组患者LPPLA2水平较正常组相比有升高,差异有统计学意义(P<0.05);非糖尿病性微血管病变组(A组)与糖尿病性微血管病变组(B组)相比,LP-PLA2水平变化,差异有统计学意义(P<0.05);Logistic回归分析提示胱抑素C、LP-PLA2、病程为糖尿病微血管病变发生的危险因素,高密度脂蛋白胆固醇为保护因素。结论LP-PLA2检测在早期糖尿病性微血管病变诊断中有重要意义。
Objective To investigate Changes and correlation of LP-PLA2 in patients with newly diagnosed type 2 diabetic microangiopathy.Methods Totally 156 cases of patients with type 2 diabetes mellitus who were treated in Department of Endocrinology from January 2020 to January 2021,were selected as the research objects,and were divided into non-diabetic microangiopathy group(Group A)and diabetic microangiopathy group(Group B),in which diabetic microangiopathy group included diabetic nephropathy and diabetic retinopathy.According to the presence or absence of diabetic microangiopathy,14 healthy subjects were selected as control group(Group C).Then,the body mass index(BMI),waist circumference,liver and kidney function,blood glucose,blood lipid,LP-PLA2,LP-PLA2,fasting C peptide,glycated hemoglobin,serum Cystatin,β2-microglobulin,urinary ACR and other clinical data of three groups of patients were codirected and compared.Results The level of LP-PLA2 in diabetic group was significantly higher than that in normal group(P<0.05),the level of LPPLA2 in non-diabetic microangiopathy group(Group A)was significantly higher than that in diabetic microangiopathy group(Group B),the difference was statistically significant(P<0.05).Logistic Regression analysis showed that Cystatin C,LP-PLA2,course of disease,and HDL were associated with diabetic microangiopathy.Conclusion The detection of LP-PLA2 has important significance in the early diagnosis of diabetic microangiopathy.
