清金化痰汤加减治疗慢性支气管炎急性发作期的效果研究

Effect of Qingjin Huatan Decoction in the Treatment of Acute Episode of Chronic Bronchitis

目的探讨痰热郁肺型慢性支气管炎患者急性发作期应用清金化痰汤加减的治疗效果。方法以2017年2月至2020年2月郑州市中医院收治的60例慢性支气管炎急性发作期患者为研究对象,通过随机数字表法分为对照组和试验组,各30例。对照组患者给予左氧氟沙星片、盐酸氨溴索注射液治疗,试验组患者给予左氧氟沙星片、盐酸氨溴索注射液加用清金化痰汤治疗,两组均治疗10 d。比较两组患者治疗前后的中医症状积分、血清免疫球蛋白A(IgA)、免疫球蛋白M(IgM)、免疫球蛋白G(IgG)水平、中性粒细胞弹性蛋白酶(NE)、组织蛋白酶G水平。结果治疗后试验组的中医症状积分显著低于对照组,差异有统计学意义(P<0.05);治疗前两组的血清IgA、IgM、IgG水平均差异无统计学意义(P>0.05),治疗后试验组的血清IgA、IgM、IgG水平显著高于对照组,差异有统计学意义(P<0.05);治疗前两组的血清NE、组织蛋白酶G水平差异无统计学意义(P>0.05),治疗后试验组的血清NE、组织蛋白酶G水平显著低于对照组,差异有统计学意义(P<0.05)。结论清金化痰汤加减与西药合用可有效缓解慢性支气管炎急性发作期患者的中医症状,改善免疫功能,抑制机体炎症反应。

Objective To investigate the effect of Qingjin Huatan decoction on acute episode of chronic bronchitis.Methods A total of 60 patients with acute exacerbation of chronic bronchitis admitted to our hospital from February 2017 to February 2020 were selected and randomly divided into control group and experimental group by random number table,with 30 cases in each group.Patients in the control group were treated with levofloxacin tablets and ambroxol hydrochloride injection,while patients in the experimental group were treated with levofloxacin tablets and ambroxol hydrochloride injection and Qingjin Huatan decoction.Both groups were treated for 10 days.The TCM symptom scores,serum immunoglobulin A(IgA),immunoglobulin M(IgM),immunoglobulin G(IgG)levels,neutrophil elastase(NE)and cathepsin G levels were compared between the two groups before and after treatment.Results After treatment,the scores of TCM symptoms in the experimental group were significantly lower than those in the control group(P<0.05).There were no significant differences in serum IgA,IgM and IgG levels between the two groups before treatment(P>0.05).After treatment,the serum IgA,IgM and IgG levels in the experimental group were significantly higher than those in the control group(P<0.05).There were no significant differences in serum NE and cathepsin G levels between the two groups before treatment(P>0.05).After treatment,the serum NE and cathepsin G levels in the experimental group were significantly lower than those in the control group(P<0.05).Conclusion The combination of Qingjin Huatan decoction and western medicine can effectively relieve the symptoms of traditional chinese medicine in patients with acute exacerbation of chronic bronchitis,improve immune function,and inhibit the body's inflammatory response.
