消化科住院患者院内交叉感染危险因素及管控方法探讨

Risk Factors and Control Methods of In-Hospital Cross-Infection Among Inpatients in Department of Digestive Diseases

目的分析消化科住院患者院内交叉感染的危险因素,探讨具体管控方法。方法对2019年1月至2019年12月在偃师市人民医院消化科住院治疗的2350例患者的临床资料进行回顾性分析,根据院内交叉感染与否将其分为感染组、未感染组,以单因素、多元Logistic回归分析法对院内交叉感染的危险因素进行分析,并探讨相应的管控方法。结果2019年1月至2019年12月期间,院内交叉感染患者92例,感染率为3.91%;感染组年龄>60岁、合并糖尿病、侵入性操作、抗生素使用时间>7 d、消化道肿瘤及超敏C反应蛋白(hs-CRP)≥10 mg/L患者比率明显高于未感染组,差异有统计学意义(P<0.05);Logistic多因素分析结果提示,年龄>60岁,差异有统计学意义(P=0.005)、合并糖尿病差异有统计学意义(P=0.006)、侵入性操作差异有统计学意义(P=0.019)、抗生素使用时间>7 d,差异有统计学意义(P=0.020)、消化道肿瘤差异有统计学意义(P=0.017)及hs-CRP≥10 mg/L,差异有统计学意义(P=0.039)为消化内科院内交叉感染的独立危险因素。结论消化科住院患者院内交叉感染危险因素较多,因此医护人员要针对性地采取预防措施,有效控制院内交叉感染的发生。

Objective To analyze the risk factors of cross infection in hospitalized patients in department of gastroenterology,and to explore the specific control methods.Methods The clinical data of 2350 patients hospitalized in the gastroenterology department of Yanshi People's hospital from January 2019 to December 2019 were retrospectively analyzed,and the patients were divided into infected and uninfected groups according to whether they had nosocomial cross-infection or not.The risk factors of cross-infection in the hospital were analyzed by single factor and multivariate Logistic regression analysis,and the corresponding control methods were discussed.Results During the period from January 2019 to December 2019,there were 92 patients with nosocomial cross-infection,and the infection rate was 3.91%,the rates of patients with age>60 years,combined diabetes mellitus,invasive operation,antibiotic use>7 d,gastrointestinal tumor and hypersensitive C-reactive protein(hs-CRP)≥10 mg/L in the infected group were significantly higher than those in the uninfected group(P<0.05).Logistic multifactorial analysis suggested that age>60 years(P=0.005),combined diabetes mellitus(P=0.006),invasive operations(P=0.019),antibiotics>7 d(P=0.020),GI tumour(P=0.017)and hs-CRP≥10 mg/L(P=0.039)were independent risk factors for nosocomial cross-infection in the gastroenterology department.Conclusion There are many risk factors of nosocomial cross-infection in inpatients of digestive department,so medical staff should take preventive measures to effectively control the occurrence of nosocomial cross-infection.
