摘要
目的 探究癌胚抗原(CEA)、糖链抗原125(CA125)、细胞角蛋白19片断(CYFRA21-1)、神经元特异性烯醇化酶(NSE)和鳞状上皮细胞癌抗原(SCC)联合检测在非小细胞肺癌(NSCLC)诊断、化疗效果评估中的应用价值。方法 选取2018年9月至2021年4月阜阳市肿瘤医院肿瘤科收治的102例NSCLC患者作为NSCLC组,所有NSCLC患者均进行化疗治疗,对NSCLC患者化疗疗效进行评价。另选取同期本院门诊收治的50例肺部良性病变患者作为良性病变组。比较两组及NSCLC患者化疗前后血清CEA、CA125、CYFRA21-1、NSE、SCC水平,比较不同病理特征、不同化疗疗效NSCLC患者血清CEA、CA125、CYFRA21-1、NSE、SCC水平;分析CEA、CA125、CYFRA21-1、NSE、SCC对NSCLC的诊断价值。结果 NSCLC患者血清CEA、CA125、CYFRA21-1、NSE、SCC水平明显高于良性病变组,差异均有统计学意义(P<0.05)。不同肿瘤直径、病理分期、病理类型中CEA、CA125、CYFRA21-1、NSE、SCC水平比较差异有统计学意义(P<0.05)。NSCLC患者化疗后血清CEA、CA125、CYFRA21-1、NSE、SCC水平明显低于化疗前,差异均有统计学意义(P<0.05)。化疗有效NSCLC患者血清CEA、CA125、CYFRA21-1、NSE、SCC水平明显低于化疗无效者,差异均有统计学意义(P<0.05)。CEA、CA125、CYFRA21-1、NSE、SCC五项联合检测对NSCLC诊断效能高于各指标单独检测(P<0.05)。结论 联合检测CEA、CA125、CYFRA21-1、NSE、SCC水平对NSCLC诊断、化疗效果评估具有重要意义。
Objectiv To explore the application value of combined detection of carcinoembryonic antigen(CEA),glycochain antigen 125(CA125),cytokeratin 19 fragment(CYFRA21-1),neuron-specific enolase(NSE)and squamous cell carcinoma antigen(SCC)in the diagnosis and chemotherapy efficacy evaluation of non-small cell lung cancer(NSCLC). Methods A total of 102 NSCLC patients admitted to the Oncology Department of Fuyang Cancer Hospital from September 2018 to April 2021 were selected as the NSCLC group. All NSCLC patients were treated with chemotherapy,and the efficacy of chemotherapy in NSCLC patients was evaluated. Another 50 patients with benign pulmonary lesions admitted to the outpatient clinic of our hospital during the same period were selected as the benign lesion group. The serum levels of CEA,CA125,CYFRA21-1,NSE,and SCC were compared between the two groups and NSCLC patients before and after chemotherapy,and the levels of serum CEA,CA125,CYFRA21-1,NSE,and SCC in NSCLC patients with different pathological characteristics and different chemotherapy effects were compared. The diagnostic value of CEA,CA125,CYFRA21-1,NSE and SCC for NSCLC were analyzed. Results The levels of CEA,CA125,CYFRA21-1,NSE and SCC in NSCLC patients were significantly higher than those in the benign lesions group,and the differences were statistically significant(P<0.05). There were significant differences in CEA,CA125,Cy FRA21-1,NSE and SCC levels among different tumor diameters,pathological stages and pathological types(P<0.05). The levels of serum CEA,CA125,CYFRA21-1,NSE and SCC in NSCLC patients after chemotherapy were significantly lower than those before chemotherapy,and the differences were statistically significant(P<0.05). The serum CEA,CA125,CYFRA21-1,NSE and SCC levels in NSCLC patients with effective chemotherapy were significantly lower than those with ineffective chemotherapy,and the differences were statistically significant(P<0.05). The combined detection of CEA,CA125,CYFRA21-1,NSE,and SCC has a higher diagnostic performance for NSCLC than the single detection of each indicator(P<0.05). Conclusion The combined detection of CEA,CA125,CYFRA21-1,NSE and SCC is of great significance for the diagnosis of NSCLC and the evaluation of chemotherapy efficacy.
