烧伤患者血清炎症因子、Ang Ⅱ、可溶性选择素及CD46变化 被引量:4

Changes and clinical significance of serum inflammatory factors,Ang Ⅱ,soluble selectin and CD46 in burn patients

摘要 目的探讨烧伤患者血清炎症因子、血管紧张素Ⅱ(AngⅡ)、可溶性选择素及CD46变化。方法选取2021年1月至12月平煤神马集团总医院诊治的60例烧伤患者(烧伤组)及60名健康体检健康人群(对照组),烧伤组根据烧伤等级再分为为两个亚组:Ⅱ度、Ⅲ度烧伤,根据烧伤皮损面积分为≤30%、>30%。检测两组及烧伤组不同烧伤等级、不同烧伤皮损面积患者血清炎症因子[白细胞介素-2(IL-2)、白细胞介素-6(IL-6)、肿瘤坏死因子α(TNF-α)、白细胞介素-1β(IL-1β)、γ-干扰素(IFN-γ)]、AngⅡ、可溶性选择素[可溶性P-选择素(sP-选择素)、可溶性E-选择素(sE-选择素)]及CD46。结果烧伤组IL-2、IFN-γ水平低于对照组,Ⅲ度烧伤患者IL-2、IFN-γ水平低于Ⅱ度患者,烧伤皮损面积>30%患者IL-2、IFN-γ水平低于烧伤皮损面积≤30%患者,差异均有统计学意义(P<0.05);烧伤组IL-6、TNF-α、IL-1β、AngⅡ、sP-选择素、s E-选择素及CD46水平高于对照组,Ⅲ度烧伤患者IL-6、TNF-α、IL-1β、AngⅡ、sP-选择素、sE-选择素及CD46水平高于Ⅱ度患者,烧伤皮损面积>30%患者IL-6、TNF-α、IL-1β、AngⅡ、sP-选择素、sE-选择素及CD46水平高于烧伤皮损面积≤30%患者,差异均有统计学意义(P<0.05)。结论烧伤患者可出现血清促炎细胞因子、AngⅡ、可溶性选择素及CD46水平的升高,并与烧伤等级及面积密切相关。 Objective To investigate the changes and clinical significance of serum inflammatory factors,angiotensinⅡ(AngⅡ),soluble selectin and recombinant cluster of differentiation 46(CD46)in burn patients. Methods A total of 60 burn patients(burn group)and 60 healthy people(control group)who were diagnosed and treated in Pingmei Shenma Group General Hospital from January to December 2021 were selected. The burn group was further divided into two subgroups according to the degree of burn.:Ⅱ degree,Ⅲ degree burns,according to the area of burn skin lesions is divided into ≤ 30%,>30%. Serum inflammatory factors[interleukin-2(IL-2),interleukin-6(IL-6),Tumor necrosis factor α(TNF-α),interleukin-1 β(IL-1β),interferon γ(IFN-γ)],AngⅡ,soluble selectin[soluble e-selectin(sE-selectin)and soluble p-selectin(sP-selectin)]and CD46 levels were detected. Results The serum levels of IL-2 and IFN-γ in the burn group were lower than those in the control group(P<0.05). Ⅲ degree burn was lower than those in patients with Ⅱdegree burn(P<0.05),burn area>30% was lower than those in patients with burn area ≤ 30%(P<0.05). The levels of IL-6,TNF-α,IL-1β,AngⅡ,sP-selectin,s E-selectin and CD46 in the burn group were higher than those in the control group(P<0.05). Ⅲ degree burn was higher than those in patients with Ⅱ degree burn(P<0.05). Burn area>30% was higher than those in patients with burn area ≤3 0%,the differences were statistically significant(P<0.05). Conclusion Serum levels of pro-inflammatory cytokines,AngⅡ,soluble selectin and CD46 were increased in burn patients,which are closely related to burn grade and area.
