miR-210、BDNF在肾上腺嗜铬细胞瘤中的表达及其临床意义

摘要 目的:分析微小RNA-210(miR-210)、脑源性神经营养因子(BDNF)在肾上腺嗜铬细胞瘤(PPC)中的表达水平,并对其临床意义进行探讨。方法:收集2011年5月-2014年12月收治的91例PPC患者(研究组)的手术标本,及35例同期健康志愿者(对照组)的正常肾上腺髓质组织标本作为对照组;采用实时荧光定量PCR(qRT-PCR)法检测组织中miR-210和BDNF表达水平并分析PPC患者中两者的相关性,评估两者对PPC术后预后的预测价值;用COX分析影响PPC患者术后预后不良发生的因素。结果:研究组患者miR-210和BDNF表达水平[(1.76±0.16)、(2.03±0.21)]较对照组[(1.00±0.00)、(1.00±0.00)]显著升高(P<0.05),且两水平呈明显正相关(r=0.726,P<0.05);不同临床病理特征PPC患者癌组织中miR-210、BDNF表达水平的差异均具有统计学意义,其中,肿瘤直径<3cm、中高分化、无淋巴结转移患者、良性肿瘤患者miR-210和BDNF水平均较低(P<0.05);Kaplan-Meier生存分析显示,miR-210和BDNF表达均与总生存时间相关。miR-210低表达、高表达组PPC患者的5年预后生存率分别为82.8%、52.2%,BDNF低表达组、高表达患者的5年预后生存率分别为87.5%、44.2%,miR-210低表达组的生存率明显高于miR-210高表达组(χ^(2)=10.854,P=0.001),BDNF低表达组的生存率明显高于BDNF高表达组(χ^(2)=21.386,P=0.000);miR-210和BDNF高表达是影响PPC发生的独立危险因素(P<0.05)。结论:miR-210和BDNF的表达水平与PPC发生及患者的临床病理特征密切相关,对PPC术后预后具有重要意义。 Objective:To analyze the expression levels of micro RNA-210(miR-210)and brain-derived neurotrophic factor(BDNF)in adrenal pheochromocytoma(PPC),and to explore their clinical significance.Methods:Surgical specimens of 91 PPC patients admitted to a hospital from May 2011 to December 2014 were selected as the study group,and normal adrenal medullary tissue samples of 35 healthy volunteers were selected as the control group.The quantitative real-time fluorescence PCR(qRT-PCR)was used to detect the expression of miR-210 and BDNF in patients with PPC,the correlation between them was analyzed,the postoperative predictive value of them in PPC patients was evaluated,and the factors affecting the poor prognosis of PPC patients was analyzed by COX.Results:The expression levels of miR-210 and BDNF in the study group[(1.76±0.16),(2.03±0.21)]were significantly higher than those in the control group[(1.00±0.00),(1.00±0.00)](P<0.05),and there was a significant positive correlation between the two levels(r=0.726,P<0.05).There was statistically significant difference in expression levels of miR-210 and BDNF in different clinicopathological features of PPC patients,and the levels of miR-210 and BDNF were lower in patients with tumor diameter<3cm,moderate and high differentiation,no lymph node metastasis,or benign tumor patients(P<0.05);Kaplan-Meier survival analysis showed that miR-210 and BDNF expression were correlated with total survival time.The 5-year survival rate of PPC patients in miR-210 low and high expression group was 82.8%and 52.2%,respectively,the 5-year survival rate of PPC patients in BDNF low and high expression was 87.5%and 44.2%,respectively,the survival rate of the miR-210 low expression group was significantly higher than that of the miR-210 high expression group(χ^(2)=10.854,P=0.001),and the survival rate of the BDNF low expression group was significantly higher than that of the BDNF high expression group(χ^(2)=21.386,P=0.000).The high expression levels of miR-210 and BDNF were the independent risk factors affecting the occurrence of PPC(P<0.05).Conclusion:The expression levels of miR-210 and BDNF are closely related to the occurrence of PPC and the clinicopathological features of the patients,which is of great significance in the postoperative prognosis of PPC.
