摘要 目的:分析慢性肾病患者营养不良的风险因素。方法:回顾性收集2018年5月-2020年6月期间某院收治的106例慢性肾病患者的临床资料,采用主观综合性营养评估方法(SGA)对患者的营养状况进行评估,将患者分为营养不良组和营养良好组,比较两组患者相关营养学指标,利用单因素分析和多因素Logistic回归分析法筛选出慢性肾病患者营养不良的风险因素,并分析相应治疗措施。结果:106例慢性肾病患者中,营养良好患者75例(70.75%),营养不良患者31例(29.25%);两组患者Hb、Alb、PA、BUN、Cr指标均存在显著差异,有统计学意义(P<0.05);单因素分析显示,患者年龄、hs-CRP、合并糖尿病、合并高血压、健康宣教均与营养不良有关(P<0.05);多因素Logistic回归分析显示,年龄≥60岁、hs-CRP≥3mg/L、合并糖尿病是慢性肾病患者营养不良的独立风险因素(P<0.05)。结论:年龄≥60岁、hs-CRP≥3mg/L、合并糖尿病是慢性肾病患者营养不良的风险因素,应根据患者实际情况制定针对性的治疗方案,降低患者营养不良发生率。 Objective:To analyze the risk factors of malnutrition in patients with chronic kidney disease.Methods:The clinical data of 106 patients with chronic kidney disease admitted to a hospital from May 2018 to June 2020 were retrospectively collected.The nutritional status of the patients was assessed by subjective comprehensive nutritional assessment(SGA).The patients were divided into malnutrition group and well nourished group.The relevant nutritional indicators of the two groups were compared.The risk factors of malnutrition in patients with chronic kidney disease were screened by univariate analysis and multivariate Logistic regression analysis,and the corresponding treatment measures were analyzed.Results:Among 106 patients with chronic kidney disease,75 cases(70.75%)were well nourished and 31 cases(29.25%)were malnourished;There was significant difference in Hb,Alb,PA,BUN and Cr between the two groups(P<0.05).Univariate analysis showed that age,hs-CRP,complicated with diabetes,complicated with hypertension and health education were all related to malnutrition(P<0.05).Multivariate logistic regression analysis showed that age≥60 years old,hs-CRP≥3mg/L and complicated with diabetes were independent risk factors for malnutrition in patients with chronic kidney disease(P<0.05).Conclusion:Age≥60 years old,hs-CRP≥3mg/L and complicated with diabetes are the risk factors of malnutrition in patients with chronic kidney disease.Targeted treatment plan should be formulated according to the actual situation of patients to reduce the incidence of malnutrition.
