摘要
目的观察不同胰岛素给药方法治疗糖尿病(DM)的临床疗效。方法100例需胰岛素治疗的DM患者,随机分为对照组和观察组,每组50例。对照组采取多次皮下注射胰岛素治疗,观察组采取胰岛素泵连续皮下输注治疗。比较两组治疗前后血糖(空腹血糖、餐后2 h血糖、糖化血红蛋白)水平、疾病治疗依从性、不良反应发生情况。结果治疗前,两组空腹血糖、餐后2 h血糖、糖化血红蛋白比较,差异无统计学意义(P>0.05);治疗后,观察组空腹血糖(7.64±1.45)mmol/L、餐后2 h血糖(9.15±2.46)mmol/L、糖化血红蛋白(6.15±2.45)%均显著低于对照组的(9.36±2.15)mmol/L、(11.36±3.58)mmol/L、(8.69±1.87)%,差异均有统计学意义(P<0.05)。观察组患者疾病治疗总依从率为98.00%,明显高于对照组的82.00%,差异具有统计学意义(P<0.05)。观察组治疗后不良反应发生率为6.00%,显著低于对照组的22.00%,差异具有统计学意义(P<0.05)。结论DM患者采取胰岛素泵给药方式治疗,不仅血糖控制效果明显,且患者治疗依从性及安全性高,临床上具有较高的应用价值。
Objective To observe the clinical efficacy of different insulin administration methods in the treatment of diabetes mellitus(DM).Methods A total of 100 DM patients requiring insulin treatment were randomly divided into control group and observation group,with 50 cases in each group.The control group was treated with multiple subcutaneous insulin injections,and the observation group was treated with continuous subcutaneous infusion(insulin pump).The levels of blood glucose(fasting blood glucose,2 h postprandial blood glucose,glycosylated hemoglobin)before and after treatment,disease treatment compliance,and occurrence of adverse reactions were compared between the two groups.Results Before treatment,there was no statistically significant difference in fasting blood glucose,2 h postprandial blood glucose,and glycosylated hemoglobin between the two groups(P>0.05).After treatment,the fasting blood glucose(7.64±1.45)mmol/L,2 h postprandial blood glucose(9.15±2.46)mmol/L,and glycosylated hemoglobin(6.15±2.45)% in the observation group were significantly lower than(9.36±2.15)mmol/L,(11.36±3.58)mmol/L,and(8.69±1.87)% in the control group,and the differences were statistically significant(P<0.05).The total compliance rate in the observation group was 98.00%,which was significantly higher than 82.00% in the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group after treatment was 6.00%,which was significantly lower than 22.00% in the control group,and the difference was statistically significant(P<0.05).Conclusion Insulin pump administration for diabetes mellitus patients not only has obvious effect of blood glucose control,but also has high treatment compliance and safety,which has high clinical application value.
