摘要
目的 探讨无创机械通气对急性左心衰竭合并心源性休克血流动力学和氧合指标的影响。方法 纳入我院2021年1月1日至2021年12月31日住院的103例急性左心衰竭合并心源性休克患者,根据患者的呼吸支持方式分为常规吸氧组(n=44)和无创机械通气组(n=59)。比较常规吸氧组和无创机械通气对心源性休克血流动力学及血气分析的影响。结果 无创机械通气组患者的呼吸困难缓解率高于常规吸氧组。经治疗后,与常规吸氧组相比,无创机械通气组收缩压更高,血管活性药物的应用时间更短,差异具有统计学意义(P<0.05)。结论 无创机械通气治疗急性左心衰竭合并心源性休克可减轻患者临床症状,改善患者氧合状况,且不会进一步加重低血压。
Objective To study the effect of non-invasive mechanical ventilation in acute left heart failure with cardiac shock(CS) on hemodynamic and oxygenation indicators. Methods 103 patients of acute left heart failure with CS were admitted to hospital from January to December of 2021. All of them were divided into oxygen supplementation group(n=44) and non-invasive group(n=59). The hemodynamic and oxygenation indicators were compared between two groups.Results There was more improvement of dyspnea in non-invasive group. Compared with oxygen supplementation group, non-invasive group was verified with hyper systolic blood pressure, less vasoactive drugs and hyper PaO, PaO/FiO(P<0.05).Conclusions Non-invasive mechanical ventilation in patients of acute left heart failure with CS can relieve clinical symptoms, improve oxygenation status.
