凝血功能指标与胰腺癌临床病理特征及预后的相关性分析

目的探讨凝血功能指标与胰腺癌患者临床病理特征及预后的相关性。方法回顾性分析50例胰腺癌患者的临床病理资料,记录患者首诊时的血浆凝血酶原时间(PT)、纤维蛋白原(FIB)及D-二聚体水平,以血浆PT、FIB及D-二聚体水平正常值上限为界将所有患者分为血浆PT、FIB、D-二聚体升高组及其对应的正常组,分析其与胰腺癌患者临床病理特征的相关性,比较各组间患者生存期的差异。结果血浆FIB、D-二聚体水平与胰腺癌患者的TNM分期、肿瘤分化程度及脉管癌栓密切相关(P<0.05)。血浆FIB、D-二聚体水平升高组患者的中位生存期均较其对应的正常组患者明显缩短,差异有统计学意义(P<0.05)。结论血浆FIB、D-二聚体水平与胰腺癌进展、病理分型、疗效、预后相关,通过密切监测凝血功能指标变化并早期给予恰当的干预,有助于提高患者的生存质量,改善预后。

Objective To investigate the correlation between coagulation function indexes and clinicopathological features and prognosis of patients with pancreatic carcinoma.Methods The clinicopathological data of 50 patients with pancreatic cancer were retrospectively analyzed,and the plasma prothrombin time(PT),fibrinogen(FIB)and D-dimer levels at the patients'first visit were recorded.The normal upper limits of plasma PT,FIB and D-dimer were used to divide the patients into elevated and normal groups.The correlation between the indicators and the clinicopathological characteristics of patients with pancreatic cancer were analyzed,and the survival of patients was compared between the groups.Results Plasma FIB and D-dimer levels were closely correlated with tumor-node-metastasis(TNM)stage,tumor differentiation and vascular tumor thrombus in patients with pancreatic cancer(P<0.05).The median survival time of patients in the groups with elevated plasma FIB and D-dimer levels was significantly shorter than that in the corresponding normal groups,with statistical significance(P<0.05).Conclusion Plasma levels of FIB and D-dimer are correlated with progression,pathological classification,curative effect and prognosis of pancreatic cancer.It is helpful to improve the quality of life and prognosis of patients by closely monitoring the changes of coagulation indexes and giving appropriate intervention in the early stage.
